The miner was on knees reaching for a wooden block to put under the equipment when pain started and it was severe enough to make miner stop.

EE was securing a load with a chain and lever binder. When applying force to lever binder to bind the chain tightly, finger got pinched in lever.

Skid loader operator was re-positioning a block (I beam) when it slid out of the bucket striking the victim as EE was walking by from behind. The beam hit EE's right ankle and was later determined at the hospital to be fractured with torn ligaments.

Truck was blocked and tire was lifted off the ground with factory scissor jack. After removing tire, truck came off jack smashing hand between the rotor and the ground.
