WHILE WALKING DOWNHILL EMPLOYEE STEPPED DOWN BUTTHERE WAS A HOLE WHICH CAUSED HIM TO LOOSE HIS FOOTAGE AND THREW HIM OFF BALANCE. HE FELL BACKWARDS AND LANDED ON HIS BACK.
WHILE DRIVING CAT 769B END DUMP DOWN AN INCLINE, EE HIT A SLICK SPOT AND LOST CONTROL OF THE TRUCK. THE TRUCK HIT A BERM CAUSING THE TRUCK TO TURN OVER. THE EE RECEIVED BRUISES ON ARM AND CHES T FROM THE TRUCK BATTERY. RAIN HAS BEEN FALLING WHICH PRODUCE THE SLICK CONDITIONS.

STATEMENTS BY THE ALLEGED VICTIM TO OTHE R EES REVEALED THIS WAS A CON GAME TO DRAW WORKMAN'S COMP. THIS IS THE REASON FOR THE LATE SUBMISSION OF THIS REPORT.
