Pond Fork Processing, LLC · Struck by flying object

EE was in the process of using a water hose to unplug material that was stuck in the heavy media line. When the unclogged material became free it came out the end of the line, striking his right hand, resulting in a fracture. Due to EE work rotation and scheduled vac during Thanksgiving week, this is a medical reportable injury only, no lost workdays.

EE's were in process of cleaning sumps and ponds along haul road with A35C Volvo articulating trucks. Stopped for lunch. After lunch injured EE backed truck from the area and traveled approx 110' backwards, striking a road berm that caused truck to roll over the embankment approx 24'. Transported to hospital. Diiagnosed with fractured ribs, clavicle, vertebra, punc lung.

Employee was sorting out structural steel moving around pieces when a beam measuring about 24"x10"x24" fell over on his left foot. He was wearing metatarsal boots, but a piece of the steel caught his foot below the metatarsar ont he side near the ankle.

EE LEFT DEEP MINE LOADING POINT IN 5TH GEAR, HE SHOULD HAVE BEEN IN 4TH. APPROX 150 FT FROM SCALE HOUSE HE TRIED TO SHIFT FROM 5TH TO 4TH, GOT THE TRUCK OUT OF GEAR, COULDN'T GET IT BACK IN GE AR SO HE DECIDED TO JUMP. NOTHING WAS WRONG PASTPRESHIFT BRAKE TEST. THE TRUCK IS AT HERITAGE EQUIPMENT FOR ESTIMATE & REPAIRS. "OPERATOR MALFUNCTION"

DRIVER STATED THAT HE STARTED OFF HILL WITH LOAD, STOPPED ON LEVEL AREA & PUT TRUCK IN 3RD GEAR,DRIVER STATED THAT HE APPARENTLY DID NOT GET SHIFT ALL THE WAY IN & IT POPPED OUT GOING DOWN THE HILL. DRIVER DECIDED TO TURN BY PASS ROAD AT HIGH SPEED, ROAD SLOPE CHANGED & THE COMBO CAUSED TRUCK TO OVERTURN. AFTER INCIDENT TRUCK UPRIGHTED & BRAKES CHECKED O-K. NOT WEARING SEAT BELT.

WHEN ASKED, HE COULD GIVE NO DATE, TIME OR ACTIVITY THAT CAUSED TH E PAIN BUT SAID HE FELT IT WAS WORK RELATED AND HAD DEVELOPED OVER THE LAST 1 OR 2 MONTHS, POSSIBLY RELATED TO SOME NECK AND SHOULDER PAIN HE PREVIOUSLY HAD THAT LATER WENT AWAY.

EE WAS WORKING ON A FLOP GATE ON THE REFUSE SCREEN DISCHARGE ON THE 4TH FLOOR. THE FLOP GATE WAS UP IN THE CLOSED POSITION TO HAVE A 2" ANGLE THAT RAN THE FULL LENGTH OF THE GATE CUT FOR REMOV AL. THE EE CUTTING THE ANGLE HAD HIS LEFT LEG IN A PINCH POINT. WHEN THE ANGLE WAS CUT FREE, THE FLOP GATE FELL, PINCHING HIS LEG.

HE FELT SOMETHING IN HIS EYE, BUT NOTHING COULD BE FOUND BY FIRST AID PROVIDER ON SITE. WAS NOT AWARE PRESCRIPTION WAS GIVEN UNTIL 12/3/02.
