Employee was cleaning the right side windows on the outside of a Komatsu WA500 front end loader, when they fell from the top of the loader and landed on the ground next to the loader.

Oldcastle SW Group, Inc · Caught in, under or between a moving and a stationary object

Conveyor belt had come to a stop, due to being overloaded with material. LOTO was performed, guarding was removed on the wing pulley. Employee used a shovel to remove material, and then they tried to scrape the rest of the material off pulley with right hand. It was at that time the weight conveyor to move backwards, causing the arm to be caught in between the belt arm, and pulley.
