Employee was removing lug nugs on stockpile truck tire using breaker bar with etension. Lug nut broke loose suddenly, causing sudden shock to wrist area, resulting in swelling and pain in Left Wrist.

Employee was performing maintenance on conveyor belt repairing a splice in belt. EE was on EE's knees, in awkward position, for a prolonged period of time. When got up felt pain in left knee. Pain continued to get worse, so was sent in for evaluation. As of time of filing full extent of injury has yet to be determined, until employee is evaluated by orthopedic specialist.

Employee was drying belt utilizing a long handled large LP torch when torch became entangled and pulled employees hand into metal guard protecting the pulley and belt. The resulting contact caused several lacerations to the left hand which required sutures. Employee was treated at local hospital and released.

EMPLOYEE WAS ATTEMPTING TO CLEAN SNOW AND ICE OUT FROM UNDER WEIGH SCALE BEAM. HE HAD TO PICK UP A STEEL PLATE TO GET UNDER SCALE. EMPLOYEE SLIPPED ON ICE AND FELL INTO OPENING AND STEEL PLA TE CAME DOWN ON LEG. SPRIN TO RIGHT KNEE.
