EE was walking to tool-house, on recently cleared snow, slipped and fell on shoulder. EE went home that night with mild discomfort. Came in the next morning wanting to get checked out.

Shoveling hopper out when hand slipped off shovel handle and stuck side of bin, causing pain and bruising in right fifth (pinky) finger. Examination showed no breaks but finger was splinted to protect finger.

Employee was exiting Skid Steer Loader. Misstepped and came down hard on heel of right foot. Sprained ankle and deep tissue bruise to right foot.

Employee was hooking up screening plant to loader hitch. While placing pin in hitch, loader shifted forward, causing pin to tilt in hitch, resulting in employees fingers being pinched between hitch pin and counterweight of loader.

Employee was using utility knife to cut conveyor belt when utility knife slipped and cut palm of left hand. Laceration required 4 stitches and tetanus shot. Employee was treated at local hospital and released with no restrictions.

EE was placing paperwork in truck that was loaded on lowboy trailer. When getting out of truck, he mis-stepped & twisted his knee. EE was able to continue to work for remainder of shift, but could not return the next morning due to swelling & pain. On 6/18/14 Dr drained fluid from knee, told EE to rest for the remainder of week & to wear knee brace until knee was completely healed.

EMPLOYEE WAS TIGHTENING SPRINGS ON SCREENING PLANT WITH A LARGE WRENCH. WHILE TIGHTENING THE SPRINGS THE EMPLOYEE TWISTED HIS BACK WHILE PUTTING PRESSURE ON WRENCH.
