Individual reported for duty on 6/6. He stated that he did not feel well. Supervisor sent for him medical treatment; no symptoms noted. Worker sent home for the day. He returned on 6/7; he felt worse. Further examination found a mild stroke prior 6/6. Individual is currently hospitalized. Report is filed as information only per MSHA-PEIR/Denver.

Worker was exiting a shop trailer at the end of the shift. Access was by a manufactured stairway system with handrails on both sides. Worker missed first step and fell approximately 4 feet to the ground (packed sand). Worker landed on his left shoulder. Experienced severe pain the following morning. Orthopaedic consultation scheduled for 4/6/9.
