EMPLOYEE WAS HAULING 1 1/4" X 0 CRUSHED STONE FROM THE CRUSHING PLANT TO THE STOCKPILE. WHILE ASCENDING THE RAMP TO THE TOP OF THE STOCKPILE, THE TRUCK OVERTURNED ON ITS SIDE, PINNING THE EMPL OYEE UNDER THE TRUCK, CAUSING FATAL INJURIES.

PLANT SUPERVISOR & EE ADJUSTED TRACKS ON DOZER. OTHER EE LEFT, & PLANT SUPERVISOR APPARENTLY STARTED TO OPERATE DOZER, IN REVERSE. HE EITHER JUMPED OR FELL OFF DOZER. DOZER STRUCK SUPERVISOR'S PICKUP, PUSHED IT OUT OF THE WAY, AND APPARENTLY RAN OVER THE SUPERVISOR, CAUSING ABDOMINAL INJURIES, FRACTURED FEMUR & PELVIS.

EE LANDEEDON HIS BUTTOCKS WHICH COMPRESSED HIS SPINE AND C AUSED A HERNIATED DISC AND PINCHED NERVES AT THEL-5 LOCATION. THE PINCHED NERVES AREA CAUSING NUMBNESS IN HIS LEGS.
