Martin Marietta Aggregates · Struck against a moving object

Spreading material on ramp to pit area. Motor on loader quit running due to low RPM & use of hydraulic controls. Operator lost use of the brakes & steering, loader traveled approx. 125' into the wall. The operators head struck the windshield & he was wearing his seatbelt during the accident.

RETURNING 4X4 WOOD MATERIAL USED ON THE FLATBED TO THE LOWER RAIL FOR TRANSPORT. A 24" TIE DOWN STRAP WAS HOOKED ON THE FRAME, WRAPPED AROUND THE WOOD, AND HOOKED BACK TO THE FRAME. WHEN THE STRAP WAS HOOKED TO THE FRAME AND STRETCHED, ITCAME UNHOOKED, HITTING THE LITTLE FINGER. OPEN FRACTURE.
