Employee bent down to pick up a wheel chock and felt a pop in his back. He was put on restricted duty on 6/12/2012.

Employee was meeting with a local Sales Rep in the office when he complained of dizziness and chest pain/racing heart. Office Manager was instructed to call for Local EMS. Employee was transported to hospital for evaluation.
Employee was assisting with the stemming of a shot. Employee finished task and reported that he was feeling ill with chest pain. Local EMS was notified and he was transported to the Hospital. This event was found to have been medical in nature and not related to mining activities.

Inspector was attempting to climb the ladder to mount the TR 70 Terex haul truck when he injured his knee. He had his left knee on the ladder and pushed himself up with the right when he heard a "pop" and felt pain in his right knee. He finished out the inspection and went to the Dr the next day and was diagnosed with a pulled muscle and put on light duty for a week.

EE opened a hatch cover in the deck of the screen platform with the intent to shovel spilled material into a chute. He walked away to pick up the shovel and when he returned, he stepped into the open hole and fell onto the deck, resulting in a cracked rib. Medical only, no lost time.

Employee performing 1000 service on 988H loader when he dropped one end of the skid plate to get better access to the transmission oil drain plug. He was removing the last of 4 bolts when the last bolt came out quicker than expected. Weight on skid plate (appx 30 lbs) slipped & corner of plate slashed employee's left thumb inside the first knuckle requiring 4 stitches.

Employee stated that he was doing a pre-shift inspection on the loader when he began to cough which caused him to slip and hurt his back. He reported this to the leadman later that morning. Reported as first aid. Began losing time on 5/7/07 allegedly due to the incident on 1/9/07.

EE was removing a pin from the excavator bucket. Pin was knocked out from other side and EE tried to hold it, but it slipped and mashed his little finger of right hand, resulting in broken bone and 3 stitches. Medical only. No lost time.

Employee removing bolts from cutting edges on 988g loader w/2 lbs hammer and knock wrench. Employee sung hammer to strike the wrench and missed hitting left hand causing a 1/4 inch gash that required 3 stitches.
Loading rock from muck pile in pit big rock hit bucket and jarred machine causing injury to back. Employee going to physician to check out. Continued having problems. Put out of work by Raleigh Orthopaedic 11/28/05. Released to return to work 12/5/05.

TWO EE WERE WORKING TOGETHER AT THE JAW CRUSHER TIGHTENING THRUST BLOCK BOLTS ON THE JAW FRAME. ONE EE TOOK THE 4' PIECE OF PIPE OFF THE WRENCH AND THE PIPE STRUCK THE OTHER EE IN HIS UPPER LI P CAUSING A OPEN WOUND.
A ROCK FELL FROM THE TOP OF THE SURGE TUNNEL FEEDER CHUTE, WHILE EE WAS CLEANING MATERIAL THAT WAS GRIDGED IN FEEDER, CAUSING A OPEN WOUND TO THE RIGHT HAND.
EE WAS STRUCK BY BACKING DUMP TRUCK WHILE HE WAS OUT OF THE BACKHOE GREASING THE MACHINE BUCKET AFTER A 5 MO. PERIOD THE ARM WAS NOT HEALING PROPERLY & SURGERY WAS REQUIRED
