WHILE CLEANING THE RETURN ROLLER ON CONVEYER, STUCK BAR UNDER BELT, CONVEYER WAS SHUT OFF BUT STILL ROLLING. BAR GOT STUCK UNDER BELT, PINCHED HIS HAND BETWEEN BAR AND CONVEYER BELT.

INJURY OCCURRED WHILE REPLACING PACKINGS IN A TELESCOPING DUMP BED CYLINDER THAT WAS REMOVED FROM TRUCK. THE FINAL PACKING HAD BEEN INSTALLED WHEN THE LAST SECTION BECAME SEIZED, WHILE TRYING TO SLIDE IT IN IT SUDDENLY FREED AND DROPPED PINCHING THE EMPLOYEE'S LEFT THUMB AND RIGHT PALM BETWEEN THE FLANGES OF THE CYLINDER.
