COMING FROM TOP LEVEL THRU TO FIRST COAL SAMPLING, STEPPED TO FLOOR FORM LADDER L ANKLE GAVE OUT
HE HAD EXITED THE LEFT DOOR OF THE OPERATOR'S COMPARTMENT AND WALKED AROUND THE PLATFORM TO THE LEFT SIDE WINDOW WHERE THE FIRE EXTINGUISHER IS MOUNTED TO CLEAN THE OUTSIDE OF THE D-10 DOZER. HE STATED HE WAS HOLDING ONTO THE CANOPY PLATFORM BANNISTER WITH HIS LEFT HAND AND A GUST OF WIND CAUSED HIM TO BE BLOWN OFF THE DOZER PLATFORM TO THE GROUND STRIKING A CHUNK OF CONCRETE APPRO
