THE 2300 VOLT POWER CABLE TO THE 200 HORSE PUMP MOTOR WAS FOUND TO BE DAMAGED A & A SPLICE.THE POUTER UNDERPROOF COVERING AT THE SPLICE APPEARED TO BE CUT AND ONE PHASE OF THE 3 PHASE CONDUCTO R FUSED TO THE EQUIPMENT GROUNDING CONDUCTOR.THIS COULD HAVE PLACED VOLTAGE ON THE PUMP HOUSING. THE VICTIM COULD HAVE CONTACTED THE PUMPING EQUIPMENT WHILE HOLDING DIFFERENT GROUNDING POTENT

Surface drill operator assisting mechanic removing internals of a 4-inch down hole hammer (off the drill). Inertia movement of internal piston caused it to knock the internal sleeve out of casing, striking driller in forehead. Driller received 3 sutures and returned to work immediately.

He stated he was having some chest pains and sweating real bad. Plant manager asked him if he needed to go to the hospital. Employee stated "yes". The Plant Manager drove him to the hospital.

THE BOLT IS COUNTERSUNK SO YOU DON'T NEED A WRENCH TO HOLD THE BOLT WHEN YOU ARE LOOSENING THE NUT. HOWEVER, EE WHENEE LOOSENED THE NUT THE BOLT CAME OFF OF THE COUNTERSUNK HOLE. EE HAD GLOVES ON SO HE PUSHED THE BOLT AND THE NUT TURNED AND CUT HIS FINGER.

EMPLOYEE WAS PUSHING GRATE ACROSS THE FLOOR INSIDE THE SCREEN TOWER. A ROCK HAD THE GRATE LODGED. WHEN HE MOVED THE ROCK, THE OTHER TWO EMPLOYEES WERE STILL PUSHING ON THE GRATE. THE GRATE BEC AME UNLODGED AND MOVED TO A POSITION THAT LODGED HIS FINGER BETWEEN THE FLOOR AND THE GRATE.

HE & ANOTHER EMPLOYEE WERE REPAIRING A LEAKING CYLINDER ON A BACKHOE AT THE PRIMARY CRUSHER. THE PIN WHICH HELD THE CYLINDER ARM IN PLACE HAD BEEN PARTIALLY CUT OUT WITH A TORCH, BUT A SMALL P IECE REMAINED IN PLACE. EMPLOYEE TOOK A PIECE OF COLD ROLL AND AN 8-LB. SLEDGE HAMMER TO TRY TO KNOCK THE PIN OUT. AS THE THE HAMMER SWUNG FORWARD, HE MISSED THE COLD ROLL AND STRUCK HIS RIGH
