EE ALLEGES SHE WAS LIFTING A 50 GALLON DRUM AND INJURED HERSELF. WE WERE NOTIFIED OF THIS INJURY 5/21/03. SHE CLAIMS INJURY TO LOW BACK AND BODY AS A WHOLE. SHE CANNOT STATE AN EXACT DATE OF I NJURY AND NOTHING WAS REPORTED LAST YEAR AT THAT TIME. WE HAVE VERY LITTLE INFORMATION ON THIS CLAIM AND SHE HAS BEEN TERMINATED SINCE 4/3/02.

ONE EMPLOYEE WAS NORTHBOUND IN HER VEHICLE ON THE QUARRY ROAD AND RAN INTO THE RIGHT PASSENGER DOOR OF ANOTHER EMPLOYEE GOING EASTBOUND. NO INJURIES WERE REPORTED, JUST VEHICLE DAMAGE. WE WERE NOTIFIED 5/21/03 THAT SHE WAS FILING A WORKER'S COMP. CLAIM FOR LOW BACK AND BODY AS A WHOLE.
