EE was walking down a ramp to a water pump. EE stepped on the hose and slipped and fell, twisting right leg. EE underwent surgery to repair multiple fractures.

Employee was straightening a conveyor belt frame. The chain broke causing the conveyor frame he was standing on to shift. He fell down across a piece of 6" channel iron breaking two ribs.

Employee was operating a trackhoe moving a log and bees came out and stung him ove 30 times on his head and arms. He proceeded to the emergency room. Tests were performed but since he is not allergic, he was released to take Benadryl and return to work.

Employee was fueling a water pump with a loader & 55 gallon drum. He raised the loader bucket with the fuel barrel inside for the gravity feed pump. He removed the wedge used to hold the barrel in place and it rolled out an pinned his sholder against the frame of the pump. Diagnosed with a contusion to the left shoulder and treated with meds and scheduled for physical therapy.

Employee was climbing out of loader and his hand slipped off the ladder and he twisted his back. Employee reported slip but refused treatment until 10/02/07. Diagnosis was just low back pain and he could return to work with no restrictions.
Employee slipped and fell down a hill and cut his right knee, requiring seven stitches.

While setting safety latch on hydraulic lift, the release lever got bumped and smashed his middle finger on his left hand between the latch and hydraulic cylinder on a Case 420 skidsteer, causing an avulsion to the soft tissue.
EE was trying to clear the shoot on the scalper and a rock bounced and smashed his little finger on his left hand, bruising it.
Employee was standing by the welding truck and the skid steer operator ran over his left foot in horesplay. He was examined but only found to have a bruise.

Employee was cleaning sand buildup under the scalper and caught his shovel on a roller. The jerking caused a pinched nerve to his right shoulder. Two visits of physical therapy have been ordered. Restricted duty until 8/30/05.
Employee was driving a haul truck & dumping a load of shot rock and caught his rt. hand & thumb in the steering wheel. He was examined & diagnosed with a contusion of his rt. hand.

PLANT WAS SHUT DOWN AND EMPLOYEE WAS WORKING ON IT. A SHOT WAS PUT OFF AND A STRAY ROCK BOUNCED UNDER THE IMPACT CRUSHER AND HIT HIM IN THE LEFT ANKLE, BRUISING IT.

EE WAS CLIMBING UP THE LADDER INTO HIS LOADER AND MISSED THE LAST STEP AND FELL-SPRAINING HIS RIGHT ANKLE.
EE WAS DRIVING A HAUL TRUCK WHEN THE BOLTS BROKE ON THE HYDRAULIC TANK UNDERNEATH AND THE WHEELS RAN OVER THE TANK, CAUSING THE TRUCK TO LOSE CONTROL AND TURN OVER. EE HAD SEATBELT ON AND WAS ONLY BRUISED AND SCRATCHED.
EE WAS CLEANING OUT IMPACTOR CHUTE WITH A SHOVEL AND HE HIT HIS ELBOW ON THE CHUTE.

EE WAS STEPPING OFF AN OFF HIGHWAY TRUCK LADDER AND STEPPED ONTO A ROCK, TURNING HIS FOOT AND CAUSING A PROBABLE ACHILLES TENDON TEAR.
EMPLOYEE WAS CLEARING TREES WITH A BULLDOZER ON THE SIDE OF A HILL. THE BANK GAVE WAY AND THE BULLDOZER ROLLED OVER ON IT'S SIDE. THE EMPLOYEE ONLY EXPERIENCED A BRUISED SHOULDER.
ON 4/12/99, EE WAS PUTTING BELTS ON THE CONE AND WHEN HE STOOD UP, HE FELT HIS KNEE POP & BEGAN TO HURT. HE REPORTED IT TO HIS SUPERVISOR BUT REFUSED MEDICAL TREATMENT. HE CAME IN ON 4/19/99 A ND REQUESTED TREATMENT.
