The operator started to empty the bucket inside the truck. While emptying it, a rock came out and entered through the window of the excavator, hitting the operator in the chest & causing an accident.
THE EE WAS WELDING A PIECE OF METAL IN THE VIBRATOR AREA ANBD MADE A SUDDEN MOVEMENT AND SLIPPERED AGAINST THE VIBRATOR FRAME AND INJURED HIS RIGHT KNEE.
EE WAS REMOVING TRANSMISSION FROM PICKUP. THE TRANSMISSION TILTED TO ONE SIDE AND THE EE IN TRYING TO AVOID FOR THE TRANSMISSION TO FALL, RESULTED WITH A SPRAINED BACK AND WAIST.

AT 11:00 AM, THE EMPLOYEE WAS OPERATING THE "TRACK DRILL" IN THE EXTRACTION AREA. HE OPENED THE DOOR TO THE CABIN WHILE THE EQUIPMENT WAS RUNNING AND THE VIBRATION CAUSED THE DOOR TO SHUT WHIL E HE HAD HIS HAND ON THE FRAME. THIS CAUSED AN INJURY TO HIS THREE FINGERS OF HIS LEFT HAND.
WAS HELPING THE MECHANIC REPAIR SHOCK ABSORBER OF ONE TEREX TRUCK. THE EMPLOYEE HAD HIS LEFT HAND UNDERNEATH THE SHOCK ABSORBER WHEN THE SHOCK ABSORBER SUDDENLY COLLAPSED FALLING ON HIS HAND.
