THE PLANT WAS SHUT DOWN. THE EMPLOYEE WAS REMOVING A BOLT TO REMOVE A GUARD WHEN HE LIFTED HIS HEAD UP STRIKING HIS HEAD ON THE BOTTOM LEFT CORNER OF THE GUARD RESULTING IN THE LACERATION.
