EMPLOYEE ATTEMPTED TO PULL SHIM FROM BENEATH HORIZONTAL BAR OF #4 SHOVEL BUCKE TWHILE THE BAR WAS BEING LIFTER WITH A BAR & CHAIN. CHAIN SLID DOWN METAL BAR ALLOWING BAR ANDSHIMS TO CLOS ON E MPLOYEE'S FINGER.
