EE WAS PRYING A ROCK LOOSE IN A REVERSE FEED BOX, WHILE REMOVING THE SMALLER MATERIAL, HE FREED THE BIG ROCK THAT WAS LODHGED IN THE BOX. THE ROCK CAUGHT EE'S LEFT INDEX FINGER.

EMPLOYEE WAS OPERATING NORTH BOUND ON A NORTH BOUND ONLY HAUL ROAD IN POURING DOWN RAIN. EMPLOYEE SAW PAYHAULER COME AROUND THE BEND TOWARD HIM, WRONG WAY, AND SWERVED RIGHT ONTO THE BERM OUT OF THE WAY. PAYHAULER DRIVER SAID HE FORGOT ABOUT THE TRAFFIC DIRECTION CHANGE ADN DROVE SOUTHBOUND. MUD PUDDLE SLPASHED OBSUCRING HIS VISION AND HE DID NOT SEE THE CAT 773 IN FRONT OF HIM.

EE WAS PERFORMING MAINTENANCE ON A CRUSHER, AFTER COMPLETING THE MAINTENANCE, EE WAS PICKING UP HIS TOOLS WHEN HE SLIPPED ON FROST WHICH CAUSE HIM TO FALL. WHILE FALLING EE TRIED CATCHING HIMS ELF WITH HIS RIGHT HAND. THIS CAUSED EE'S WRIST TO BREAK.

THE WIND BLEW A DOOR SHUT AT THE LANDFILL GARAGE AND EE'S RIGHT HAND RING FINGER WAS IN THE DOOR JAM. THE DOOR SMASHED HIS FINGER, EE DID FIRST AID ON HIS OWN FINGER 7 THOUGHT IT WOULD E ALRIG HT, BUT HE THEN WENT TO THE DR ON THE 12 WHEN HE CAME BACK HE HITIFIED ME OF GOING.
