Employee was operating a skid steer on an small drainage berm when the machine become over balanced and turned on its side causing minor damage. Case was reported on 3-31-17 to the EHS department and the case become an reportable injury on 4-1-17.

Employee was repairing an axle trunnion and while setting it on the ground, left hand middle finger became trapped between the part and a bolt causing fracture and laceration to finger.
EE was cutting on a seam on a convey belt making repairs. He was cutting the belt with a box knife. He was cutting the belt towards him when the knife slipped off the belt hitting him in the right knee causing a small laceration to the knee.
EE WAS GETTING OUT OF BED OF POWDER TRUCK. HIS FOOT SLIPPED OFF OF STEP AND HE STRUCK HIS FACE ON THE SIDE OF THE TRUCK CUTTING HIS NOSE AND LIP. HE REQUIRED APPROX. 15 STITCHES FOR 60TH WOUND S.

EE ATTEMPTED TO ENTER A MANLIFT WHICH WAS PARKED UNDER A CRUSHER FEED BIN. OTHER WORKERS WERE CUTTING A PIECE OF 1/2" SHEET STEEL TO MODIFY THE CRUSHER FEED BIN. HE WAS NOT WEARING A REQUIRED HARD HAT. WHEN HE WALKED UNDER THE CRUSHER FEED BIN, A PIECE OF STEEL FELL, STRIKING THE LEFT SIDE OF HIS SCALP. HE HAD 5 STITCHES AND WAS RELEASED TO WORK IMMEDIATELY.
HE WAS WALKING THROUGH THE OFFICE TRAILER WHEN THE HEEL OF HIS BOOT CAUGHT ON A STEP AND HE WRENCHED HIS KNEE. HE DID NOT REPORT THE INJURY UNTIL 4-15-96.

EMPLOYEE CLIMBED THE STEPS TO START THE PRIMARY CRUSHER ENGINE. WHEN HE STEPPED ON THE STEP TO START THE ENGINE, HE SLIPPED ON AN ICY STEP AND FELL SIX FEET TO THE GROUND.
EMPL STANDING UNDER CHUTE OF ROCK CRUSHER AS CHUTE WAS BEING REPLACED.WHEN CHUTE WAS RELEASED TOOK FULL WEIGHT OF CHUTE TO HOLD IT IN PLACE.WEIGHT OF CHUTE CAUSED RIGHT KNEE TO GIVE WAY.
INDIVIDUAL WAS LOADING "ANFO" ONTO 74 FORD F600 POWDER TRUCK, SLIPPED ON LADDER AND SPRAINED RIGHT KNEE. X-RAYS WERE NEGATIVE. AFTER THREE (3) DAYS, FOLLOW-UP VISIT WAS NEGATIVE.
