EMPLOYEE WAS DESCENDING THE STEPS OF THE #2 CATWALK. HE SLIPPED ON THE TOP STEP, HIS FEET WENT OUT FROM UNDER HIM. EMPLOYEE LANDED ON HIS RIGHT ELBOW. HE WAS UNABLE TO CATCH BECAUSE HE WAS NOT HOLDING THE RAILING.

EE DESCENDED STEPS ON CONVEYOR #2 TO THE GROUND, THEN AFTER PLACING BOTH FEET ON GROUND TOOK A STEP AND STEPPED ON A ROCK CAUSING HIM TO FALL AND TWIST KNEE.
EE WAS CLEANING AROUND A CONVEYOR SUPPORT, EE FAILED TO SEE THE CONCRETE PIER & HER CAUGHT THE BOTTOM LEFT SIDE OF BUCKET ON THE CONCRET PIER, CAUSING EQUIPMENT TO STOP SUDDENLY.
