EMPLOYEE WAS HAMMERING ON A SMALL BOARD, LAID ACROSS TH END OF A SMALLER PIPE TO BE FIT INTO A BIGGER PIPE. WHILE HAMMERING, WITH ARM OUT STRETCHED, HE PINCHED A NERVE IN HIS NECK, PULLED TEND AND TORE MUSCLE.

EE WAS SETTING A 5\X7\ CRUSHER SCREEN OVER A RAILING TO THE GROUND APPROX 10\ BELOW. THE SCREEN WENT DOWN FASTER THAN ANTICIPATED BY THE EE. THUS PULLING HIM & BENDING THE RAILING SO THAT HE F ELL WITH THE SCREEN OVER THE EDGE TO THE GROUND.
