Employee was picking up a backpack that contained a folding knife that had inadvertently deployed. When the employee grasped the backpack, the knife cut the distal phalanx of the right index finger, requiring stitches.

EE knelt to inspect track pads. EE raised up, felt pop in knee. Knee popped a 2nd time, pain left. Ee cont working. Knee popped a 3rd time. EE called Supv. EE was diagnosed w/a sprain. MRI was ordered at followup visit. EE was diagnosed with Meniscal tear on 06/08/20. EE can no longer cont w/normal work duties. EE sent home pending medical evaluation. 1st shift missed 06/19/20

The excavator walked in front of the truck, then reached over and put the bucket in the bed of the truck and began to walk backwards, pulling the truck out. When the truck was freed, the truck came forward, striking the windshield of the excavator. The truck struck the walking levers of the excavator which caused it to walk forward. Excavator then pushed truck til it went off.

EE was fueling his loader. He stepped up on a step that runs along the fuel containment area & grabbed the fuel nozzle. When he stepped off the step, his left foot hit the ground & the ankle rolled inward. EE went to the ground in pain & called his supervisor. He was taken to the hospital. He says the foot just did not hit the ground level.
