I.NAME CHECKING 3" PUMP THAT HAD QUIT PUMPING.TRIED TO STRAIGHTEN DISCHARGE HOSE,IT CAME LOOSE FROM PUMP THROWING HOT WATER ON HIS ARMS AND LEGS.

EMPL WAS DISASSEMBLING WINDOW CRANK ASSEMBLY ON A FORD WATER TRUCK THE SPRING LOADED PARTS SPRANG LOOSE AND CUT UPPER R THUMB
WHEN EMPLOYEE WAS MOVING DRAG ACROSS THE ROAD, THE MACHINE CAME OUT OF THE TRACK THEN SLIPPED ON THE DRIVING BLOCKS. WHEN THE MACHINE JUMPED TE TRACKS, IT CAUSED THE EMPLOYEE TO BE JOLTED.
EMPLOYEE WAS WALKING BACK FROM THE 6" PUMP WHEN HIS FEET HIT APIECE OF ICE CAUSING HIM TO SLIP AND FALL STRIKING HIS RIGHT ARM AND RIGHT LEG ON THE GROUND.

EMPLOYEE WAS HA6LING ROCK AND STOPPED TO HAVE LUNCH. WHEN GETTING DOWN FROM CAB HE WAS HOLDING HIS DINNER BUCKET AND THERMOS AND FLASHLIGHT IN LEFT HAND. HIS FOOT SLIPPED ON STEP AND HE HIT LEFT SIDE OF MO6TH ON HAND RAILING.
THE EMPLOYEE AL5EGES THAT HE SLIPPED AND FELL ON A PIECE OF SHALE,LANDING ON HIS RIGHT HAND.WHILE WALKING TO THE DRAGLINE.
