MIDDLE BRIDGE STARTED MOVING UP WHICH CAUSED BAC K BRIDGE TO BEGIN TO SWIVEL. BACK BRIDGE OPERATOR TRIED TO REENTER HIS DECK AND GOT CAUGHT BETWEEN THE DECK AND RIB.

MAN WAS CLEANING U SPILLAGE AT #1 BET TAIL ROCK DISLODGED FROM TOP BETWEEN ROOF BOLTS STRICKING HIS SHOULDER TRYING TO GET AWAY FROM ROCK HE TRIPPED OVER TAIL ANCHOR JACK. HE CAUGHT HIS FEET ON JACK TWISTING IT WHILE FALLING WE ASSUMED IT TO BE BROKEN AND TREATED IT AS SUCH.
