CLIMBING EUCLID TRUCK TURNED TO LOOK AT CONE BECAUSE IT WAS FILLING UP. SLIPPED & FELL DOWN THREE(3) STEPS ON LEFT SIDE.
EMPLOYEE WAS SHOVELING A GRAVEL PILE ON THE BELTUNDER THE SCREEN WHEN HE JAMMED HIS HAND ON THE STEEL BEAM. HE WAS WEARING GLOVES.

HE WAS CLIMBING OUT OF THE EUCLID TRUCK, SLIPPED ON THE STEPS AND FELL ON HIS KNEE. THE STEPS WERE WET AND MUDDY WHICH CONTRIBUTED TO THE INJURY. THE RIGHT KNEE WAS BADLY BRUISED AND SWOLLEN.

EE SLIPPED WHILE CLEANING ROCKS OFF WALKWAY OF HYDRAULIC GRIZZLY, PUTTING HANDS OUT TO CATCH HIS BALANCE IN TURN PLACING HIS HAND UNDER GRIZZLY.
EMPLOYEE WAS SQUATTING DOWN TO PICK UP A BLOCK, WHEN HE ATTEMPTED TO GET UP FROM A DEEP SQUAT POSITION HE FELT SHARP PAIN IN LEFT KNEE. EMPLOYEE ATTEMPTED TO TAKE CARE OF INJURY HIMSELF TO NO AVAIL. AFTER A WEEK OR SO PAIN AND SWELLING CONTINUED, HE CONSULTED, HE CONSULTED WITH A DR.10-18-94 AND WAS SEEN AGAIN A WEEK LATER, AT WHICH TIME SURGERY WAS SCHEDULED.
