EE WAS DRIVING THE DUMPSTER STATES HE HIT A LARGE BUMP HE DIDNT SEE. THIS CAUSE HIM TO LEAVE THE SEAT AND HIT HIS HEAD ON ROOF OF CAB. SAID HE TOLD HIS FOREMAN EXPLAIN HE PAIN IN HIS NECK AND FOREMAN TOLD HIM TO GO TO THE DR.

EMPLOYEE STATED IT WAS A DRY WIDY DAY AND FELT SOMETHING GET IN HIS EYE, THOUGHT HE GOT IT OUT. BY 11:00 AM HE FELT PRESSURE BEHIND HIS EYE AND STARTED GETTING A BAD HEADACHE. FINISHED OUT THE DAY AND WENT HOME. CONTINUED TO WORSEN WHEN HE WENT TO THE DOCTOR.
