While repairing a damaged trunion tire, the employee needed to trim a piece of rubber in order to get the puller installed. The box knife being used slipped lacerating the employee's left ring finger.

Employee was attaching a trailer mounted pump to a track hoe bucket in order to move the pump into position. The track hoe bucket unexpectantly lowered, catching the employee's hand between the bucket and pump frame.

While treating mine water at Outfall 2, hydrated lime in the hopper escaped through a gap at the flow adjustment plate and hit the employee in the eyes. Employee immediately began flushing eyes with bottled water. Due to severe irritation, the employee was taken in for medical evaluation.

Fall caused a laceration to EE right eyebrow, fracture of his cheek bone, and a contusion to his right shoulder. He also scratched his right hand while attempting to arrest the fall. After Med evaluation 11/15/13 EE needs surgery
