AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO. 1 ENTRY AT 6 ROOM IN THE 7 RIGHT 4 SOUTH SECTION. THE FALL IS ABOUT 10' HIGH 15' WIDE AND 16' LONG. THE FALL WAS ABOVE THE BOLT ANCHORAGE BUT DID EFFECT VENTILATION. NO INJURIES OR ENTRAPMENT.
MAIN LARGE PERSONNEL ELEVATOR (20 PASSENGER) AT 3 NORTH SHAFT STOPPED IN SHAFT WITH ONE EMP ABOARD. IT WAS FOUND THAT THE OVER MOTION INTERLOCK SWITCH ON TOP OF ELEVATOR HAD TRIPPED (SAFETY DE VICE FOR EMERG BRAKING) POSSIBLY BY EMERG. BUTTON BEING BUMPED WHILE ELEVATOR WAS IN MOTION.
AS THE INJURED WAS ENTERING THE LONGWALL FACE FROM THE HEADGATE, HE STRUCK HIS SHOULDER ON THE END OF A PRESSURE HOSE THAT WAS HANGING ON THE NO.1 SHIELD.
WHILE CUTTING A PIECE OF WOOD ON A TABLE SAW, THE INJURED ACCIDENTALLY CUT THE TIPS OF HIS MIDDLE AND RING FINGERS ON HIS LEFT HAND.
ROOF FALL OCCURRED ON #6 MAIN BELT APPROX 50' OUTBY STATION MARKER #83. FALL MEASURED 10'H, 17'W, 20'L. VENTILATION WAS NOT AFFECTED. NO PEOPLE WERE IN AREA. ROOF HAD BEEN SUPPORTED WITH 5' CO NVENTIONAL BOLTS AND 5"X7"X14' TIMBERS TO ROOF.
EE WAS THROWING CRIB BLOCKS (5"BY6"X24") ACROSS STAGE LOADER ON THE 6 FT LONGWALL SECT APPROX 20' INBY 7RM WHEN HE CAUGHT HIS FINGER BETWEEN THE CONVERYOR SIDE BOARDS AND A CRIB BLOCK. THE ROOF WAS SPAWLING AND THE CRIBS WHERE TO BE USEDAS ROOF SUPPORT
THE INJURED WAS IN THE PROCESS OF SHOVELING A COAL SAMPLE INTO A PLASTIC SAMPLE BAG WHEN HE FELT SOMETHING GIVE OUT IN HIS LOWER BACK FOLLOWED BY A SEVERE PAIN.
AN UNINTENTIONAL ROOF FALL OCCURRED AN THE 2 MAIN BELT ALONG MAINS WEST HAULAGE ONE BLOCK OUTBY STAMARKER 22. FOUR FEET OF MATERIAL CAME DOWN ON THE CONVEYOR FOR A DISTANCE OF 15 FT IN L AND 7 FT W, THE ENTRY IS 17 FT WIDE. VENTILATION NOR PEOPLE WERE EFFECTED. CONVENTIOANL 3 FT ROOF BOLT HAD BEEN USED IN THIS AREA WITH NO SUPPLEMENTIAL SUPPORTS.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 6-RIGHT OFF 4 SOUTH HEADINGS, #1 ENTRY- 7 ROOM INTERSECTION. THE FALL MEASURED 20' L X 20' W. THE HEIGHT IS ABOVE THE ANCHORAGE OF 7' POINT ANCHOR BO LTS BUT AS YET CAN NOT BE MEASURED. VENTILATION WAS NO AFFECTED. THE FALL HAS BEEN DANGERED OFF. NO PEOPLE WERE AFFECTED. THE LOCATION IS OUTBY THE ACTIVE SECTION.
A ROOF FALL OCCURRED SHORTLY AFTER A LONGWALL SET-UP ENTRY WAS CUT THROUGH INTO A STANDING ENTRY. THE SET-UP ENTRY WAS DRIVEN FROM 7 RIGHT 4 SOUTH TO 6 RIGHT 4 SOUTH. THE CUT THROUGH FELL IN F IRST AND 250' FELL HOURS LATER. THERE WERE NO INJURIES OR ENTRAPMENTS. VENTILATION WAS NOT AFFECTED.
EE WAS IN THE PROCESS OF INSTALLING A ROOF BOLT WITH A ROOF BOLT MACHINE IN THE FACE OF NO. 5 ENTRY IN THE 4 SOUTH SECTION. WHILE INSTALLING A ROOF BOLT THROUGH A STEEL STRAP, A PIECE OF ROCK, 5'X2'X5" THICK FELL, PARTIALLY STRIKING HIM ON THE HEAD AND NECK.
EE WAS SPREADING CALCIUM ON THE 7-RIGHT OFF 4 SOUTH SUPPLY TRAM-ROAD, BETWEEN 13 AND 20 ROOM. WHEN EE LIFTED A FULL BAG (50 LB.) HE FELT PAIN INHIS BACK.
INJURED WAS IN THE PROCESS OF TRAMMING A ROOF BOLTING MACHINE FROM THE OPERATORS COMPARTMENT. DURING THIS PROCESS, THE EE DANGLED HIS FOOT OUTSIDE OF THE COMPARTMENT ND GOT IT SQUEEZED BETWEEN THE MACHINE AND A LARGE PIECE OF SLATE.
EE WAS RESETTING THE POWER ON THE LOAD CENTER AT #12 ROOM, #2 ENTRY OF THE 6RIGHT LONGWALL SECTION. THE CONDITION OF THE AREA WAS NORMAL TO DRY,7' HIGH. EE KNELT OVER AND WHEN HE STOOD BACK UP , HE FELT PAIN IN HIS RIGHT CALF.
A ROOF FALL OCCURRED ON THE NO. 3 MAIN BELT CONVEYOR ENTRY. THE FALL IS FROM 0 TO 3 FT HIGH, 16 FT WIDE AND 25 FT LONG. DID NOT EFFECT VENTILATION, CAUSE ANY ENTRAPMENT OR INJURIES. IT FELL AB OVE BOLT ANCHORAGE.
EE WAS WALKING ALONG A BELT CONVEYOR AT A BELT DRIVE WHEN HE SLIPPED ON SOME WET, UNEVEN BOTTOM AND TWISTED HIS RT. KNEE.
THE INJURED STOPPED ALONG THE HAULAGE ROAD TO ASSIST ANOTHER EMPLOYEE TO LIFT AND HANG A 20' OINT OF 3' WATER PIPE (PLASTIC) IN THE PROCESS, THE INJURED FELT A PAI IN HIS LOWER BACK.
UNINTENTIONAL ROOF FALL OCCURRED AT THE INTERSECTION OF #50 CROSSCUT, 7 ENTRY IN THE 4 SOUTH SECTION. THE INTERSECTION FELL ABOUT 10' HIGH ABOVETHE ANCHORAGE OF THE BOLTS. VENTILATION WAS NOT AFFECTED, NOR WERE ANY EMP'S INJURED OR ENTRAPPED. AREA WAS ORIGINALLY BOLTED WITH 7' POINT ANCHOR BOLTS.
A MAN WAS TRAPPED ON THE 3 NORTH SHAFT ELEVATOR FOR APPROXIMATELY 1 HOUR AS A RESULT OF 3 BLOWN FUSES. THE ELEVATOR COMPANY CAME AND CHANGED THE FUSES AND FREED THE TRAPPED EMPLOYEE.
AN UNINTENTIONAL FALL WAS FOUND IN THE 7-RIGHT SECTION AT #21 ROOM. IT EXTENDED FROM THE #1 ENTRY LT RIB INTO THE CROSSCUT TOWARDS #2 ENTRY, A DISTANCE OF 60 FEET. THE HEIGHT WAS APPROX. 18' A BOVE THE COAL AND 17' (THE CROSS-CUT WIDTH). THE #1 ENTRY FACE WAS INBY 100'. VENTILATION WAS AFFECTED UNTIL IT WAS CUT THRU.
WHILE RIDING ON AN OPEN STYLE PERSONNEL CARRIER (JEEP) NEAR STA. MARKER #42 ON MAINS WEST TRACK HAULAGE, EMP STRUCK HIS HEAD ON A LOW TIMBER BECAUSE HE WAS NOT FACING THE DIRECTION OF TRAVEL.
THE INJURED WAS CHANGING A EHV MOTOR ON A SHIELD SUPPORT. AFTER REMOVING THE SCREWS ON THE MOTOR. HE TURNED THE WATER PRESSURE TO THE SHIELD SLOWLY, SO THE MOTOR WOULD DISLODGE. THERE WAS NO P RESSURE BECAUSE THE MAINGATE PUMPS WERE OFF HE FORGOT TO CLOSE THE PRESSURE VALVE OFF AGAIN. A LITTLE WHILE LATER THE PUMPS WERE TURNED ON. THE MOTOR FLEW OFF THE SHIELD STRIKING HIM ON THE B
WHILE REMOVING BELT STRUCTURE, THE INJURED WAS TAKING OUT A BELT ROLLER. HIS FOOT SLIPPED ON A WET ROCK AND HE PULLED SOMETHING IN HIS BACK.
EE WAS GETTING UT OF A BATTERY POWERED UNIHAULER WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK AND DOWN HIS RIGHT LEG.
EE WAS IN THE PROCESS OF CARRYING TWO 5 GALLON CANS OF OIL, WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER.
WHILE WASHING SOME MUD FROM UNDER A BELT TRANSFER, THE INJURED CLOSED THE WATER VALVE ON A WATER HOSE AND IT BURST. THE PRESSURE JERKED THE HOSE FROM HIS HANDS, STRIKING HIM ON THE STOMACH AND LEFT WRIST.
THE INJURED WAS REMOVING 7200 V CABLE FROM THE CABLE CLAMPS ON THE TRACK ENTRY. WHILE IN THE PROCESS, HIS FOOT SLIPPED AND HIS GLOVE CAUGHT ON THE CABLE CLAMP, JERKING HIS RIGHT MIDDLE FINGER.
THE MAIN LARGE PERSONNEL ELEVATOR AT 3NORTH SHAFT STOPPED IN THE SHAFT WITH ONE EMPLOYEE ABOARD. A BRIEF ELECTRICAL STORM HAD BLOWN TWO SMALL CONTROL FUSES OF THE ELEVATOR. WITHIN 35 MINS IT W AS BACK IN OPERATION AND THE EMPLOYEE OUTSIDE, UNINJURED.
EMP WAS ENROUTE TO MAKE AN EXAMINATION IN THE MINE. HE STOPPED HIS TRACK JEEP TO GET OFF TO THROW A SWITCH. AS HE STEPPED FROM THE JEEP HIS FOOT PARTIALLY HIT A TIE AND A HOLE, THUS TWISTING HIS LEG. THE AREA WAS AT THE 2 RT. OFF 4 SOUTH TRACK SWITCH, HEIGHT 7', DRY CONDITION.
EE WAS OPERATING A RAMCAR ON THE 7-RT. OFF 4 SOUTH SECTION. THE BOTTOMS WERE WET AND IRREGULAR. AS HE WAS TRAMMING FROM THE FACE TO THE FEEDER HE HIT A HOLE OR QUICK DIP IN THE BOTTOM AND THE RAMCAR BOUNCED. THE QUICK JOLT CAUSED PAIN TO OCCUR IN HIS NECK AND BACK.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE RIGHTRETURN ENTRY, #8 OF THE 4 SOUTH MAINS @ 30 ROOM.FALL WAS ABOVE AN OLD OVERCAST. FALL PREVENTS TRAVEL OF THIS RETURN IN ITS ENTIRETY, BUT DID NOT AFFECT VENTILATION. IT FELL AT AN INTERSECTION, APPROX 10'H. THERE WERE NO INJURIES OR ENTRAPMENT.
THE INJURED WAS ASSISTING SEVERAL OTHER EE LOAD A 995 BOLT ELECTRICAL CABLE ONTO A SUPPLY CAR. WHILE ATTEMPTING TO DROP THE CABLE ONTO THE SUPPLY CAR. THE CABLE STRUCK HIS RIGHT HAND CAUSING I T TO STRIKE THE SUPPLY CAR COUPLER.
EMP WAS HOSING DOWN THE WALKWAY AND STRUCTURE ALONG #1 RAW COAL BELT WHEN HE STEPPED ON BROKEN GLASS FROM A LIGHT FIXTURE. THE GLASS PIERCED HISBOOT (IRON AGE RUBBER BOOT) AND PUNCTURED HIS LE FT FOOT.
EE WAS STEPPING DOWN FROM LADDER OF TEREX PAN, AS HE DID HIS FOOT LANDED ON TWO ROCKS. HIS WEIGHT CAUSED THE ROCKS TO KICK OUT AND TWIST HIS ANKLE.
AN UNINTENTIONAL ROOF FALL OCCURRED IN #3 ENTRY STAGLOADER OF 5-RIGHT JUST INBY 7RM. THE FALL MEASURED 50'LX10'W AND THE HIEGHT IS UNDETERMED. IT EXTENDED FROM 7 RN INBY TO THE FACE. VENTILATI ON WAS NOT OBSTRUCTED. ALTHOU THE STAGE LOADER AND CRUSHER ARE COVERED UP IT IS STILL OPERATIONAL.
THE ROOF IN THE LW CONNECTORS IN 6-RIGHT BEGAN TO WORK AT THE FACE AROUND 12:00 NOON. THE CREW SET CRIBS IN THE INTERSECTIONS BUT COULD NOT HOLD IT. IT FELL FROM THE FACE OF #2 ENTRY, WHERE IT HAD JUST HOLED THRU INTO 5-RIGHT, OUTBY A DISTANCE OF 230 FEET (EXCEPT THE INTERSECTION). VENTILATION NOT AFFECTED, THE HEIGHT 12-15', WIDTH 18'.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #1 ENTRY, 4 S HEADINGS AT SURVEY STA 6968. IT MEASURED 18'W, 18'L, & 10'H. THE OGIGINAL SUPPORTS WERE7' POINAT BOLTS. ALTHOUGH THIS IS IN THE RETURN AIR COURSE. VENTILATION WAS NOT AFFECTED.
EE WAS ACTING FOREMAN ON THE LONGWALL. AFTER TALKING WITH HEADGATE OPERATOR HE TURNED TO WALK AWAY & HIT HIS HEAD ON A ROOF STRAP THAT WAS BENT DOWN A LITTLE. THIS OCCURED NEAR SURVEY STA 7402 IN #3 ENTRY OF 5-RT OFF 4-SOUTH. NORMAL HEIGHT IS 72" TO 84" & IT WAS DRY.
EMPLOYEE WAS INSTALLING TRACK RAIL (60LB) IN THE 6 RIGHT OFF 4 SOUTH HEADINGS. #10 ROOM, DRY CONDITIONS. EMPLOYEE PUT THE POINT OF A SLATE BAR INTO A BOLT HOLE OF A 30' RAIL AND TRIED PUSHIN G ON IT TO ROLE HIM ON THE RIGHT SIDE OF HIS FACE.
EMPLOYEE WAS CARRYING A PIECE OF BELT STRUCTURE (STAND) FOR THE 4 SOUTH BELT WHEN HE TRIPPED OVER A PIECE OF SRATE LYING ON THE MINE FLOOR. THE AREA WAS BETWEEN #3 AND #4 RM. CONDITIONS WERE D AMP AND NORMAL HEIGHT 6'. EMPLOYEE STRAINED CALF MUSCLE IN RIGHT LEG.
AN UNINTENTIONAL ROOF FALL OCCURRED IN #1 ENTRYOF 4-RIGHT (TAILGATE) AT STATION MARK 1900 AND INBY FOR 20 FEET. THE FALL EXTENDED THE WIDTH OF THE 18' WIDE ENTRY. THE HEIGHT IS UNDETERMINED. V ENTILATION WAS NOT AFFECTED. OBSTRUCTED TAILGATE PLAN WAS PUT IN FORCE. THE AREA WILL BE MINED THRU.
WHILE TRAMMING HIS BATTERY POWERED UNIHAULER, THE INJURED STRUCK A RUT IN THE TRAM ROAD AND BOUNCED UP AND STRUCK HIS HEAD ON THE CANOPY, HAMMING HIS NECK.
AN UNSAFE ROOF CONDITION DEVELOPED AT THE TAILGATE OF THE LONGWALL AT STATION MARKER 21+65 AND OUTBY 20 FEET. MAKIGN IT NECESSARY TO MPLIMENT THE PLAN FOR TAILGATE UNSAFE TO TRAVEL. THE ROOF WAS BREAKING UP BETWEEN THE CRIBS AND SLOUGHING OFF HTE RIBS. THE AREA'S WAS MINED THRU BY THE END OF THE SHIFT. VENTILATION WAS NOT AFFECTED.
THE INJURED WAS ASSISTING A SCOOP OPEPRATOR LOAD A BELT TAKE-UP UNIT ONTO A SUPPY CAR. AS THE SCOOP OPERATOR WA SRETRACTING THE BLADE OF THE SCOOP. THE BLADE PINCHED THE INJUREDS LEFT HAND BE TWEEN THE RETRACTABLE BLADE AND THE SCOOP BUCKET.
AN UNINTENTIONAL ROOF FLAL OCCURRED N #1 ENTRY OF 4-RIGHT (TAILGATE) AT 23+00 AND OUTBY 20'. THE FALL COVERED THE WIDTH OF THE 18' W INTERSECTON AND THE HIEGHT WAS UNDETERMINED. VENTILATION WAS NOT AFFECTED BECAUSE THE ROOF BASICALLY SAT DOWN ON THE CRIBS, TAPPERING IN TOWARDS THE GOB. THE TAILGATE IMPASSABLE PLAN WAS INSTITUED AND THE FALL WA SMINED THRU.
THE INJURED WAS TRYING TO POSITION THE MIDDLE SECTION OF THE BELT TAKE-UP UNIT, USING A PROTABLE RATCHET WINCH, TO LINE UP WITH END SECTION. WHILE RATCHETING. THE MIDDLE SECTION SLIPPED OFF ITS LEG STANDS AND DROPPED ON THE INJUREDS RIGHT FOOT.
WHILE MOVING THE MINING MACHINE FROM ONE CUT TO ANOTHER, THE INJURED PICKED UP THE TRAILING CABLE TO MOVE IT OFF TO THE SIDE, AS HE DID HE FELT A SHARP STINGING PAIN TO HIS LEFT KNEE.
AN UNINTENTIONAL ROOF FALL OCCURRED IN #1 ENTRY OF 4-RIGHT (TAILGATE) AT 27+00 AND OUTBY FOR 20'. THE FALL COVERED THE WIDTH OF THE 18' WIDE ENTRY AND THE HEIGHT UNDETERMINED. VENTILATION WAS NOT AFFECTED. THE TAILGATE IMPASSABLE PLAN WAS INSTITUTED. THE AREA WAS MINED THRU THE SAME SHIFT.
THE INJURED WAS IN THE PROCESS OF REMOVING THE CONTINUOUS MINER TRAILING CABLE FROM A HOOK LOCATED ONT EH SIDE OF THE MINING MACHINE. DURING THIS PROCESS THE CABLE SLIPPED FROM HIS LEFT HAND, CAUSING A PULLED STRAIN TO HIS RIGHT ARM. THE INJURED DID NOT START MISSING WORK UNTIL 2-28-96.
THE INJURED WAS LOADING JUNK INTO HIS UNI-HAULER. WHILE ATTEMPTING TO THROW A PIECE OF SCRAP ANGLE IRON ONTO THE UNI-HAULER, IT SLIPPED FROM HISHAND & SMASHED THE LITTLE FINGER OF HIS RT. HAND BETWEEN THE SCRAP & UNI-HAULER.
UNINTENTIONAL ROOF FALL OCCURRED IN THE MAIN RETURN #8 ENTRY AT #30 CROSSCUT AT SURVEY STATION 7340 IN 4 SOUTH. FALL IS 50'L, 17'W, 15'H. VENTILATION WAS AFFECTED. NO INJURIES OR ENTRAPMENT. INBY SECTIONS ARE IDLED TIL VENT. REESTABLISHED
AN UNINTENTIONAL ROOF FALL OCCURRED IN #1 ENTRY OF 4-RIGHT (TAILGATE ENTRY) AT STATION PLUS 31+50 AND CONTINUING OUTBY APPROX. 20'. IT FELL THE WIDTH OF THE 18'ENTRY AND THE HEIGHT IS UNDETERM INED. THE VENTILATION WAS NOT COMPLETELY AFFECTED. THE EMERGENCY TAILGATE IMPASSABLE PLAN WAS INSTITUTED. THEY WILL MINE THROUGH THE FALL.
EE WAS WALKING IN THE 2 TRACK ENTRY OF 5-RT OFF 4 SOUTH HEADINGS WHERE AREA IS DRY WITH PERIODIC IRREGULATITIES OF THE BOTTOMS. HE EXPLAINED THAT HE TWISTED HIS FOOT AND AS HE CONTINUED TO WAL K IT GRADUALLY GOT TO FEELING WORSE. HE FINALLY HAD TO TAKE HIS WEIGHT OFF IT ALTOGETHER.
AN UNINTENTIONAL ROOF FALL OCCURRED AT 13 ROOM (TRACK ENTRY) INTERSECTION OF THE 5 RIGHT OFF 4 SOUTH HEADINGS. THE FALL MEASURED 2'L X 18'W X UNDETERMINED H.). THE INTERSECTION HAD BEEN BOLT ED WITH 7' PT ANCHOR BOLTS AND CRIBBED IN BOTH CROSS-CUTS. THE AREA IS DRY. THE FALL DID NOT AFFECT VENTIATION OR WORKERS.
EE WAS OPERATING A REMOTE CONTROL MINER IN #2 ENTRY OF THE 6 RIGHT SECTION OFF 4 SOUTH. AS HE BACK THE MINER FROM THE RIGHT SIDE OF THE FACE TO FINISH CLEANING UP THE LEFT SIDE, A PIECE OF SLA TE ROLLED FROM HIGH ON THE RIB COAL AND PUSHED THE EE UP AGAINST A WHEEL OF A RAMCAR WHICH WAS WAITING TO BE LOADED.

THE ROOF BEGAN TO SQUEEZE AND DETERIORATE JUST OUTBY THE INTERSECTION AT 39 +25.A DISTANCE OF APPROX 15" AND THE WIDTH OF THE 18' ENTRY.IN THE 4 RT ENTRY.THE LONGWALL SHEARER HAD JUST CUT OUT AT THE TAIL APPROX 20 MINUTES EARLIER.IT WAS STILL PASSABLE HOWEVER,UNSAFE,TO TRAVEL PLAN WAS INITIATED.
THE INJURED WAS IN THE PROCESS OF ASSISTING IN LIFTING STEEL BEAMS ONTO AN OVERCAST THAT WAS BEING ERECTED WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
AN UNINTENTIONAL ROOF FALL OCCURRED AT THEH 13 ROOM INTERSECITON OF 5-RIGHT BELT ENTRY. IT MEASURED 18' W 20'L AND 9'H. THE INB SECTION WAS IDLE FOR A LONGWALL MOVE. VENTILATION WAS NOT DIS RUPTED. 7' PT ANCHOR BOLTS HAD BEEN USED WITH CRIBBING INT EH CROSS CUT.
STEEL PLATE HAD BEEN CUT FROM THE SLATE CHUTE ON TOP OF THE #3 PORTAL TIPPLE. EE WAS MOVING A PIECE OF STEEL FROM THE WALKWAY (APPROX 2'X4' X 3/4") BY HAND, WHEN HE FELT PAIN BETWEEN HIS SHOUL DER BLADES. THE AREA WAS WET FROM WASHING DOWN BUT THIS HAD NO EFFECT ON THE INCIDENT.
AN UNINTENTIONAL ROOF FALL OCCURRED AT NO. 3 CROSSCUT, BETWEEN NO. 1 AND NO.2 ENTRY IN THE 6 RIGHT 4 SOUTH SECTION. THE FALL WAS 60 FT. LONG, 16 FT. WIDE AND 8 FT. HIGH. IT DID NOT DISRUPT VEN TILATION. NO INJURIES OR ENTRAPMENT. THE ORIGINAL SUPPORT WAS 7 FT. POINT ANCHOR ROOF BOLTS.
EMPLOYEE WAS AT THE END OF THE TRACK HAULAGE IN THE 4-RT. LW SECTION LOADING DINATRACK CHAIN ONTO A FLATCAR. AS HE LIFTED THE CHAIN BY HAND HE FELT SEVERE PAIN IN HIS RIGHT ARM.
THE INJURED WAS IN THE PROCESS OF CLEANIGN MUD FROM THE CAT PADS OF A BULL DOZER WITH A SHOVEL. THE SHOVEL GOT STUCK AND WHILE TRYING TO FREE IT, IT CAME BACK AND STRUCK HIM ON THE RIGHT SIDE OF HIS FACE.
EE WAS JACKING NEW SHEARER ON SET UP ENTRY FOR NEW LONGWALL. AS HE JACKED THE JACK WAS NOT CAUGHT GOOD ENOUGH AND SLIPPED FROM ITS POSITION. AS IT FELL IT CAUGHT EE RT HAND AND PINNED IT BETWE EN THE JACK AND RANGING ARM.
THE INJURED WAS IN THE PROCESS OF BUILDING A STOPPING. WHILE LIFTING A BLOCK, HE FELT APAIN IN HIS RIGHT LOWER BACK. HE DID NOT START MISSING WORK UNTIL 12-5-95.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO.2 LONGWALL SET UP ENTRY OF THE 5 RIGHT 4 SOUTH SECITON. THE FALL IS ABOUT 60 FT LONG 17 FT WIDE AND 10 FT HIGH. VENTILATION WAS NOT AFFECTED NOR W ERE THERE ANY INJURIES OR ENTRAPMENT.
AN UNINTENTIONAL ROOF FALL OCCURRED AT AN INTERSECTION ON THE 2 MAIN BELT ENTRY AT LOCATION MARKER NO. 28. THE FALL WAS 16' WIDE, 16' LONG, AND 8' HIGH. VENTILATION WAS NOT AFFECTED NOR WERE A NY MINERS ENTRAPPED OR INJURED.
EE WAS ROOF BOLTING WITH A ROTARY DRILL IN THE 5 RT. SECTION ENTRY OF THE LW CONNECTOR ENTRY STATION 4+80. THE AREA IS DRY WITH DAMP BOTTOM AND 90" HIGH. WHEN REMOVING HIS DRILL STEELS FROM A FRESHLY DRILLED HOLE, HE PULLY ONLY 1 STEEL OUT.THE OTHER ONE STUCK IN THE HOLE BUT THEN FELL OUT AND LANDED ON HIS LT. HAND.
A FALL OCCURRED ON THE TAILGATE ENTRY OF THE 4 RIGHT 4 SOUTH LONGWALL SECTION. THE FALL WAS JUST IN FRONT OF THE TAILGATE SHIELDS. VENTILATION WAS NOT AFFECTED NOR WERE ANY MINERS ENTRAPPED. T HE FALL WAS MINED THROUGH IN A SHIFT.
AT 6:30 AM THE BIG ELEVATOR WAS OBSERVED ON THE BOTTOM OF THE SHAFT WITH THE DOOR OPEN, BUT WOULD NOT OPERATE. THE ELEVATOR REPAIR CO WAS CALLED IN AND THEY CHANGED A BLOWN FUSE ON THE DRIVE U NIT. REPAIRED AT 9:30AM. ELEV EMPTY. NOONE ON IT
AN UNINTENTIONAL ROOF FALL OCCURRED INT HE #1 RETURN ENTR OF 5 RIGHT OFF 4 SOUTH HEADINGS THE FALL DISRUPTED THE VENTILATION BUT DID NOT AFFECT PEOPLE. IT MEASURED 17'W 22'L AND 10'H. ORIGI NALLY THIS AREA WAS BOLTED WITH 7' COMBINATION PT. ANCHOR BOLTS. THE SECITON INBY (013) IS IDLE UNTIL CLEANED UP.
EMPLOYEE WAS LOADING 60 LB MIN RAIL ONTO A RAIL-CAR IN THE 2 RIGHT OFF 4-SOUTH SECTION. HE JACKED A RAIL UP AND BLOCKED IT, BUT NOT GOOD ENOGUTH. IT FELL OFF THE BLOCK AND STRUCK HIM ON THE H EEL. CONSITIONS IN THIS AREA WERE NONRMAL, WITH NO SPECIFIC HAZARDS THAT STOOD OUT.

AN UNINTENTIONAL ROOF OCCURRED ON THE 2 SOUTH HAULAGE APPROX 50' INBY STATION SURVEY PT 6477 AND INBY TO JUST PAST #5 ROOM. THIS IS AN IDLE AREA OF THE INE WHICH ONLY EXAMINERS HAVE TO TRAVEL . THE FALL MEASURE 50' IN LENGTH, 17'W AND THE HEIGHT UNDETERMINED BUT IN EXCESS OF THE 4' CONVENTINAL BOLTS USED, VENTILATION NOT AFFECTED.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 3 MAIN BELT CONVEYOR AT STA. MARKER 47. IT MEASURED 40'L X 17'W X 5'H. IT WAS ORIGINALLY SUPPORTED WITH 4' CONVENTIONAL BOLTS AND CRIBS. VENTILATION WAS NOT AFFECTED. NOONE INJURED OR TRAPPED.
THE MID-NIGHT SHIFT LAMPMAN WASHED DOWN THE BIG ALTERNATE ESCAPEWAY ELEVATOR. HE FAILED TO CLEAN MUD FROM THE DOOR TRACK. WHEN HE FINISHED HE SENT ELEVATOR TO BOTTOM BUT IT FALIED TO REPORT BA CK TO THE SURFACE BECAUSE OF THE DOOR FAILURE. IT WASN'T NOTICED RIGHT AWAY. BACK IN SERVICE - 625 AM.
AN UNINTENTIONAL ROOF FALL ON OCCURRED ON THE 3 MAIN BELT CONVEYOR AT STA MARKERS 45 AND OUTBY FOR A DISTANCE OF APPROX 80'. THE FALL WAS ABOVE THE ANCHORAGE OF CONVENTIONAL 4' ROOF BOLTS AND WAS APPROX. 5' HIGH. THE WIDTH WAS THE WIDTH OF THE ENTRY (17'). VENTILATION WAS NOT AFFECTED. NO ONE ENTRAPPED.
THE ROOF AT THE LONGWALL TAILGATE BECAME UNSAFE TO PASS THRU DUE TO DETERIORATING ROOF & HEAVING BOTTOMS AS IT ADVANCED THRU THE INTERSECTION AT STA PLUS 17+89 VENTILATION IS NOT INTERUPTED & NO ONE HAS BEEN TRAPPED THE DISTANCE THRU THIS BAD ROOF IS APPROX 16 FEET
EMP WAS ORKING ON THE 3 MAIN BELTLINE HELPING TOREMOVE BELT STRUCTURE. THE AREA IS 4-5' HIGH ANDDAMP TO DRY. AS HE BENT OVER AND STARTED TO PICK UP ON A 42" BELT STRUCTURE HE FELT PAIN IN HIS LOWER BACK AND COULD NOT STRAIGHTEN UP.
WHILE INSPECTING A CONVEYOR BELT, THE INJURED STEPPED ON SOME UNEVEN BOTTOM AND INJURED HIS LEFT FOOT.
AN UNINTENTIONAL ROOF FALL OCCURRED ON THE TAILGATE OF THE 4 RIGHT 4 SOUTH LONGWALL SECITON, 1 BLOCK INBY SURVEY SPAD 7053. THE FALL OCCURRED ABOVE THE BOLT ANCHORAGE. VNETILATION WAS NOT AF FECTED NOR WAS THERE ANY ENTRAPMENT OR INJURIES.
THE INJURED WHO IS CLASSIFIED AS A REPAIRMAN/WELDER WAS FILLING IN AS A RIVERMAN WHILE WALKING ALONG THE DECK OF A COAL BARGE THE INJURED TRIPPED ON A ROPE & FELL ON HIS LEFT KNEE
THE INJURED WAS WALKING THROUGH THE SHIELDS OF A LONGWALL FACE WHEN HE STEPPED ON SOME LOOSE COAL AND TWISTED HIS LEFT KNEE THEN FELL ON THE SAME KNEE. THE INJURED MISSED NO WORK UNTIL 10-6-95 AT WHICH TIME HE UNDER WENT SURGERY.
THE INJURED WAS IN THE PROCESS OF REMOVING A BROKEN WATER SPRAY FROM A CONTINUOUS MINING MACHINE. WHILE EXERTING HIS WEIGHT ON A WRENCH, THE WRENCH SLIPPED CAUSING HIS BODY TO TWIST ABRUPTLY. THE INJURED FELT A SHARP PAIN IN HIS LOWER BACK.
WHILE WALKING TO HIS ASSIGNED JOB, THE INJURED STEPPED IN A SMALL POTHOLE IN THE ROADWAY. THIS RESULTED IN A SWOLLEN RIGHT KNEE AND HAMSTRING PULL.

AS HE WAS CLEANING IT OUT THE DRILL SSTEEL FELL FROM THE HOLE AND STRUCK HIM ON THE TOP OF HIS LEFT HAND.

EE WAS PULLING MINER CABLE BY HAND. AS HE PULLED WITH PRESSURE ON LEFT KNEE, HE FELT A SHARP PAIN. THIS HAPPENED ON THE 5 RT SECTION IN THE CROSS CUT BETWEEN 2 & 3 ENTRY OF 7 ROOM. BEGAN MISSI NG WORK 1-4-96.
INJURED WAS WORKING ON THE TIGHT SIDE OF THE 6 MAIN BELT TAKE UP. A PIECE OF ROCK 2' X 2' X 8HAD FALLEN FROM THE RIB. THE BELT BUT WHEN HE TRIED PUSHING IT ONTO THE BELT. HE JAMMED HIS SHOULDER.
THE LARGE ALTERNATE ESCAPEWAY ELEVATOR USED REGULARLY TO TRANSPORT MEN PORTAL TO PORTAL FAILED TO REPORT TO THE SURFACE. THE CAUSE WAS DIRT BUILT UP IN THE TRACK OF THE DOOR'S WHICH WOULD NOT PERMIT THE DOORS TO LOCK. THIS WAS CLEANED AND PUT BACK IN SERVICE AT 9:45 PM.
EXAMINING BELTLINE EMPLOYEE WAS CROSSING OVER PREVIOUSLY FALLEN AREA. HE WAS COMING DOWN OFF FALL HE SLIPPED ON WET SLATE AND STRAINED RIGHT ANKLE.
EMPLOYEE WAS ROOF BOLTING ON THE 5RT SECTION AT THE FACE, WHEN A PIECE OF RIB BROKE OFF AND ROLLED ONTO HIS FOOT. (8" X 3').
AN UNINTENTIONAL ROOF FALL OCCURRED IN #3 ENTRY BETWEEN #11 & 12 ROOMS OF THE 4 SOUTH MAIN HEADINGS. THIS IS ALSO THE INTAKE ESCAPEWAY ACTION WAS TAKEN TO RE ROUTE ESCAPEWAY. THE FALL MEASURED 100 LONG X 18WIDE X 10 HIGH THE ROOF HAD BEEN SUPPORTED WITH 7' PT ANCHOR BOLTS AND T-5 CHANNELS.

IN THE #27 RM. OFF #6 ENTRY OF 4 SOUTH SECTION. THE MACHINE OPERATOR (VICTIM) WAS CUTTING ROCK BOTTOM WITH THE CONTINUOUS MINER, WHEN A PIECE OF ROCK (1"X2") FLEW FROM THE BITS AND STRUCK EE O N HEAD.
EE WAS OPERATING A RAIL JEEP AND FAILED TO LOWER HIS HEAD FOR A STEEL BEAM, THEREFORE HE HIT HIS HEAD. THE AREA IS IN AN IDLE PART OF THE MINE AT #4 ROOM OVERCAST ON 2 SOUTH HAULAGE.
EMPLOYEE WAS EXAMINING 2 MAIN BELT ON A PRESHIFTEXAM. AS HE CROSSED ROCK A FALL AT 25 STA. MARKER HE STEPPED ON UNEVEN ROCK AND TWISTED HIS ANKLE.
REMOVING A TOP PAN FROM #7 RAMCAR. IN THE PROCESS OF LIFTING THE PAN OFF HIS LEFT ELBOW STRUCK A CANOPY LEG.
EMPLOEYE STATED THAT HE BENT OVER AND PICKED UP 2 BUNDLES OF ROOF BOLT PLATES (APPR 30LBS).WHEN HE DID HE COULDN'T STRAIGHTEN UP DUE TO PAIN IN HIS LOWER BACK.
EMPLOEYE WAS REPLACING A TOP CRADLE ROLLER ON A 42" BELT-LINE AS HE PULLED THE ROLLER TOWARD HIMSELF HE FELT A SHARP PAIN IN HIS MID-LOWER BACK.
WHILE CLEANING UP SCRAP METAL FROM A MAGNET AREA INJURED CLAIMS HE FELL OUT OF A WINDOW ONTO A SCRAP PILE AND ROLLED DOWN OVER THE SCRAP PILE.
EMPLOYEE WAS INSTALLING A DRAW-BAR FOR A BELT CAR IN-PLACE-OF A COUPLER ON A LOCOMOTIVE. WHEN HE WAS INSERTING THE PIN BACK IN THE LOCOMOTIVE THE LOCOMOTIVE WAS MOVED TO ALINE THE HOLES BETTER , BUT HE FAILED TO MOVE HIS HAND OUT OF THE WAY.HE PINCHED HIS FINGER UNDER THE PIN.
THE LARGE ALTERNATE ESCAPE WAY ELEVATOR WAS DOWN FROM 6 AM UNTIL 6:30 PM. SINCE THE ELEVATOR HAD BEEN DOWN DURING THE NIGHT THE SAFETY DIRECTOR WAS TESTING IT TO BE SURE IT WAS OPERATIONAL. WH EN THE BOTTOM WAS PUSHED IT TRAVELED UP 10FT AND STOPPED. NO ONE WAS ON ELEVATOR AT TIME.
THE LARGE ALTERNAE ESCAPEWAY ELEVATOR WAS DOWN FROM 11:26 PM UNTL 2:15AM 2-16-95. THE ELEVATOR TRAVELED BEYOND THE TOP LANDING AND CAME TO A STOP. ONE MAN WAS STUCK ON THE ELEVATOR FROM 11:2 6 PM UNIL 0005 2-16-95. 39 MIN.
THE MAIN ELEVATOR (LARGE) WAS DOWN FROM 9:25 AM TO 11AM THE FELEVATOR WENT BELOW BOTTOM LANDING AND WOULD NOT REPORT BACK SURFACE. A BAD CONTACTOR HAD TO BE REPLACED.
THE MAIN ELEVATOR [LARGE] WAS DOWN FROM 7:30AM TO 9:00 PM. THE ELEVATOR WENT ABOVE IT'S LALNDING & CAUGHT ON ITS SAFETY CATCHES. IT HAD TO BE HOISTED UP TO RELEASE THE CATCHES & THEN MANINTAEN ANCE WAS PERFORMED ON THE MACHINE DRIVE SHEAVE & HOIST CABLES. THE CONTROLS & SWITCHES WERE ALSO CHECKED.
THE INJURED WAS IN THE PROCESS OF WALKING ALONG THE GUNNEL OF A COAL BARGE ON THE RIVER. HE SLIPPED ON SOME ICE AND HIS FOOT STRUCK A MANHOLE COVER CAUSING IT TO FLIP IN THE AIR. WHEN HE FEL L, HE LANDED ONT HE MANHOLE COVER TAHT STRUCK HIM IN THE GROIN. HE INCURRED A LARGE LUMP BEHIND HIS TESTICLES.
THE ALTERNATE SMALL ELEVATOR WHICH IS PART OF THE PRIMARY INTAKE ESCAPEWAY WAS DOWN FROM 130 AM TIL 300 AM DUE TO VERY COLD TEMPERATURE THE DOORS WOULD NOT SHUT ALL THE WAY. THE DOOR LINKAGE W AS LUBRICATED AND REAPRIED ALL EE WERE NOTIFIED TO USE AN ALTERNATE ESCAPE ROUTE.
THE MAIN LARGE ELEVATOR WAS FOUND IN OPERATABLE DUE TO A MALFUNCTION IN THE TOP LIMIT SWITCH. ALL PEOPLE UNDERGROUND WERE NOTIFIED OF PROBLEM. ELEVATOR CO WAS NOTIFIED AN WAS REPAIRED AT 1115 PM 2-4-95.
THE EMPLOYEE WAS ABOUT TO SET CRIBS AT THE LOCATION OF LONGWALL SHIELD RECOVERY. HE CUT THE BAND FROM THE PALLET OF CRIB BLOCKS. WHEN HE DID THIS THE CRIB BLOCKS SLID OFF AND ONE STRUCK THE VA LVE OF THE PRESSURE RELIEF HOSE FOR THE SHIELDS. THE END OF THE HOSE ( A HIGH PRESSURE HOSE) FLEW UP AND STRUCK EMPLOYEE IN THE EYE AND NOSE.
EMPLOYEE WAS WASHING DOWN THE SIDES OF AN UNDERGROUND COAL STORAGE BIN WITH A PRESSURE HOSE A SPLASH OF WATER CONTAINING SMALL PARTICLES ENTERED HIS EYE
AN UNINTENTIONAL ROOF FALL ABOVE THE ANCHORAGE ZONE OF 5' FOOT RESIN ROOF BOLTS WAS FOUND BY EXAMINER ON HIS RUN. THE FALL MEASURED AT LEAST 80' LONG 18' WIDE AND 7' HIGH. IT OCCURRED IN THE W ORKED-OUT 2 SOUTH MAIN HEADINGS BETWEEN 3 & 5 ENTRIES EXTENDING IN THE CROSS CUT FROM STATION MARKERS 3289 & 3292.
EMPLOYEE WAS BEGINNING TO WELD WITH AN ELECTRIC WELDER. HE STRUCK THE WELD ROD TO THE METAL PREMATURELY WITH OUT HIS HOOD DOWN. HE THOUGHT HE SUSTAINED FLASH BURNS TO THE EYE BUT LATER A PIECE OF METAL WAS FOUND. HE DID NOT MISS WORK AT FIRST. EMPLOYEE BEGAN MISSING WORK 2-2-95.
THE MAIN [LARGE] ELEVATOR WENT DOWN OR WOULD NOT REPORT TO THE BOTTOM AT APPROX 6AM. COLD WEATHER CONTRIBUTED TO THE DOORS NOT OPENING & CLOSING. HOWEVER THE ELEVATOR REPAIRMAN DID REPLACE A S AFETY SWITCH IN THE HOISTWAY. MEN WERE NOTIFIED & INSTRUCTED TO USE ALTERNATE RTE.
AN UNINTENTIONAL ROOF FALL OCCURRED ABOVE THE ANCHORAGE OF 5 1/2 DOUBLE LOCK BOLTS ON THE 3 HEADINGS BETWEEN 1 & 2 ENTRIES IN 30 ROOM CHARGING STATION. THE FALL MEASURED 85' LONG 18' WIDE 10' HIGH. VENTILATION WAS NOT OBSTRUCTED. NO ONE WASINJURED OR ENTRAPPED. THE FALL WAS ALSO OUTBY THE WORKING SECTION. A BATTERY WAS COVERED BUT WAS RECOVERED.

INJURED WAS WALKING INTO THE SECTION AFTER GETTING OFF THE MANTRIP. AS HE WAS WALKING HE STEPPED IN A RUT AND TWISTED HIS RT ANKLE.
THE SECONDARY ELEVATOR USED FOR AN ALTERNATE ESCAPE ROUTE FROM THE MINE WENT DOWN AT 3:45 PMM. THE WEIGHT CHAINS WERE ADJUSTED AND IT WAS PLACED BACK IN SEVICE AT 7:30 PM. ALL MEN HAD BEEN N OTIFIED OF THIS AND THERE ISTRUCTIONS FOR ESCAPE.
THE MAIN AND SECONDARY ELEVATORS WENT DOWN DURING THE NIGHT AND MEN HAD TO BE RE-ROUTED TO THE DRIFT OPENING OF THE MINE. COLD WEATHER (BELOW 10 DEGREES F) CONTRIBUTED TO THE FAILURE. THEY WERE BOTH OPERATING AGAIN, ONE -555AM THE OTHER 730 AM.

EE WAS A PASSENGER IN A MANTRIP. AS THE MANTRIP PASSED THRU A STEEL HAULAGE DOOR, THE UNATTENDED BEGAN TO CLOSE. THE OPERATOR STOPPED TO REACH OUT TO PUSH THE DOOR BACK OPEN, BUT BEFORE HE COU LD, THE MANTRIP DRIFTED BACKWARDS AND THE BOTTOMCORNER OF THE DOOR CAME INTO THE PASSENGERS' COMPARTMENT AND SPEARED THE FOOT OF VICTIM.
EE WAS TRYING TO PUSH A HYDRAULIC JACK INTO PLACE WITH HIS LEGS, BUT DOING SO HE PULLED MUSCLE IN HIS LOWER LEFT BACK.
A ROOF FALL OCCURRED AT SURVEY STA. 5015 ON THE #3 MAIN BELT HAULAGE. IT ORIGINATED IN THE INTERSECTION AND EXTENDED 60' IN LENGTH 16 FT WIDE AND THE HEIGHT IS ABOVE THE ANCHORAGE APPROX 10'. 5' CONVENTIONAL BOLTS AND CRIBS WERE ORIGINAL INSTALLED AT SUPPORTS.
AN UNINTENTIONAL ROOF FALL OCCURRED AT SURVEY STATION 5437. IT MEASURED 40 FT LONG 16 FT WIDE AND APPROX. 10 FT HIGH. THE ROOF FALL ABOVE THE ANCHOURAGE ZONE OF THE 5 FT CONVENTIONAL BOLTS NO PEOPLE OR VENTILATION WERE AFFECTED.
EMPLOYEE WAS ATTEMPTING TO USE A KNIFE TO SHAVE WOODEN PUMP PART. THE KNIFE SLIPPED AND CUT HIS LEFT HAND.

AN UNINTENTIONAL ROOF FALL OCCURRED AT AN INTERSECTION OF NO. 20 CROSSCUT NO. 1 ENTRY IN 2 RIGHT 4 SOUTH THIS IS THE RETURN AIRWAY FOR THE 2 RIGHT 4 SOUTH SECITON APPROX 1500 FT OUTBY THE FACE S. VENTILATION WAS INTERUPTED AND MINING WAS CEAAN THE FALL. FALL WAS 17 FT WIDE X 25 FT LONG X 18 FT HIGH.
AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #2 ENTRY(RETURN) OF THE 4 SOUTH SECTION. IT MEASURES APPROXIMATELY 520' LONG, EXTENDING FROM 50' INBY SURVEY STA. 6767 TO SURVEY STA. 6870. THE HEIGHT VARIES FROM 15 TO 18 FT HIGH WHICH IS 1-3FT ABOVE THE ANCHORAGE OF 7' POINT ANCHOR BOLTS.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE NO 1 RETURN ENTRY (12 ROOM) AT SURVEY SPAD 6883. THE ROOF WAS BAD DURING DEVELOPMENT. SUPPLEMENTAL LONGER BOLTS WITH STEEL CHANNELS AND 7 CRIB WERE I NSTALLED DURING DEVELOPMENT. RETURN AIR WAS REDUCED BUT THE SECTION WAS IDLE AT THE TIME OF THE FALL.

EE WAS HELPING OTHER MECHANICS LIFT AND SET IN PLACE BELT CONVEYOR DRIVE UNIT STRUCTURE 8 INCH BEAMS AT ONE POINT HE TOOK ON MORE WEIGHT THAN HE SHOULD HAVE AND FELT PAIN IN RIGHT ELBOW.
EE WAS PLATERING A FREE STANDING BLOCK WALL THAT HAD BEEN LAID DRY AND WAS TO BE USED AS AN OVERCAST WALL. THE BASE OF THE WALL WAS NOT ON SOLID FOOTING IT BEGAN TO LEAN AND THEN FELL ON THE W ORKERS.
EE WAS PLASTERING A FREE STANDING BLOCK WALL BLOCKS 8X8X16 SOLID THAT HAD BEEN LAID DRY AND WAS TO BE USED AS AN OVERCAST. THE BASE OF THE WALL WAS NOT ON SOLID FOOTING IT BEGAN TO LEAN AND TH EN FELL ONT HE EE.

THE INJURED WAS ASSIGNED TO HAUL SUPPLIES FROM ONE SURFACE STRUCTURE TO ANOTHER WHILE WALKING TO HIS TRUCK, HE SLIPPED ON SOME ICE AND FELL ON HIS LT WRIST. THIS WRIST HAS A STEEL PLATE FROM A PREVIOUS ACCIDENT. THE INJURED HAS NOT MISSSED ANY WORK AS OF THIS TIME.
THE INJURED WAS IN THE PROCESS OF PREPARING TO ROOF BOLT ON TOP OF A FALL. AS HE WAS WALKING ON THE FALL HE STEPPED ON LOOSE ROCK AND TWISTED HIS RIGHT KNEE.
WHILE ATTEMPTING TO RELEASE A RATCHET TYPE HAND WINCH AT THE RIVER LOADING FACILITY THE WINCH SLIPPED FROM A TOOTH CAUSING IT TO SPIN WITH FORCE. THE HANDLE OF THE WINCH STRUCK THE INJURED ON HIS RIGHT WRIST CAUSING A FRACTURE.
THE INJURED WAS ASSISTING ANOTHER EE INSTALL ROOF BOLTS WITH A HAND HELD AIR DRILL. WHILE ATTEMPTING TO PRESSURIZE THE DRILL AGAINST THE ROOF, THE DRILL STEEL BROKE AND STRUCK THE INJURED ON B OTH HANDS. THE INJURED SUSTAINED A LACERATION ONHIS RT THUMB AND A BROKEN MIDDLE FINGER ON HIS LEFT HAND.

EMPLOYEE WAS LIFTING CONVEYOR ROLLER OUT OF MUD WHEN HE FELT PULL IN HIS BACK.
AN UNINTENTIONAL ROOF FALL OCCURRED ABOVE THE ANCHORAGE ZONE OF THE ROOF BOLTS IN THE INTAKE ESCAPEWAY 2 BLOCKS INBY THE 3 NORTH SHAFT SURVEY SPAD 2433. THE FALL WAS CONFINED TO THE INTERSECTI ON AND FELL JUST BARELY ABOVE THE 7 FT CONVENTIONAL BOLTS.
EMPLOYEE WAS PICKING UP SLATE SPILLAGE AND THROWING IT ON THE CONVEYOR BELT. A PIECE OF SLATE FLEW BACK STRIKING HIM IN THE EYE.
EMPLOYEE WAS PUTTING PIN INTO COUPLING DEVICE ON LOCOMOTIVE & DRAW BAR WHEN THE PIN WOULDNT GO IN HE HAD OPERATOR BUMP LOCOMOTIVE CONTROL TO GIVE SLACK BUT WHEN HE DID THE PIN CAUGHT HIS FINGE R AGAINST LOCOMOTIVE BUMPER
UNINTENTIONAL ROOF FALL OCCURRED ABOVE THE ACHORURAGE ZONE OF THE ROOF BOLTS 7 FT COMBO IN THE 1-RT OFF 4 SOUTH SECTION. THE FALL OCCURRED IN THE INTERSECTION MARKED BY SURVEY STA 5702 AND CON TINUED INBY AT TOTAL DISTANCE OF 110 FT TOWARDS STA 6627 THE FALL RANGED FROM 15 TO 18 FT WIDE AND 18 FT IN HEIGHT.
EMPLOYEE WAS TRIMMING A FEW COPPER STRANDS FROM A CABLE WITH A UTILITY KNIFE AND CUT HIS RIGHT HAND WHEN THE KNIFE SLIPPED WHILE PULLING TOWARD HIMSELF.
