Weather forecasts predicted an excessive rain event, indicating a potential for the Red River to flood. The plant manager initiated emergency action planning, removing mobile equipment from flood areas and the shutdown of electrical power to the plant. All personnel were also made aware of the situation. On May 1, 2025 the Red River flooded, preventing access to the Waurika Plant.

Employee reported that he was clearing a jam on top of sand plant when his foot slipped and he fell. He reported that he hit his head, right shoulder and right jaw. He reported throbbing in his head. Blurred vision and speech was slightly sulrred.

SLIPPED GOING DOWN WALKWAY TO GREASE WATER PUMP. RAILS WERE PROVIDED BU T HE WAS NOT USING THEM. STRAINED AND BRUISED RIBS AND DIAPHRAGM.
