Employee was getting meter reading from a truck. As he turned and stepped away from the truck, he tripped and fell, catching himself with his left hand. He sustained a servere contusion to his wrist. He was placed on modified duty on 2/29/08 after receiving medical attention.

EE states that he has been having problems with his hands getting numb while operating machinery. He also claims to have pain in his elbows and arms. He has been diagnosed with carpel tunnel in both hands and is under medical care. EE went on restricted duty on 4/24/2006.

EE WAS DIGGING OUT A TRENCH WITH THE EXCAVATOR. HE WAS MOVING THE EXCAVATOR BACKWARDS WITH THE TURNTABLE TO HIS LEFT, WHICH WAS THE WRONG POSITION. THE WEIGHT OF THE EXCAVATOR CAUSED THE TRACKS TO FALL INTO THE TRENCH ON ITS SIDE. EE IS HAVING BACK PAIN. (EXCAVATOR ENDED UP ON ITS SIDE; EE WAS NOT EJECTED FROM CAB - WAS WEARING SEAT BELT.)
EE WAS EMPTYING A CORE BARREL BY STRIKING IT AGAINST A WOOD BLOCK TO DISLODGE THE SAMPLE FROM THE BARREL. WHILE DOING SO EMPLOYEE FELT PAIN IN HIS RIGHT WRIST. EMPLOYEE HAS STRAINED LIGAMENTS.
EMPLOYEE WAS RAISING THE HOOD ON THE JOHN DEERE BACKHOE. EMPLOYEE STATES THAT AS HE WAS LIFTING, THE HOOD CAUGHT WHILE BEING RAISED AND HE FELT PAIN IN HIS RIGHT SHOULDER.
EE WAS LIFTING AN AUGER INTO THE RACK OF THE DRILL RIG. HE WAS LIFTING WITH ONE ARM. ANOTHER EE HAD THE OTHER END OF THE AUGER. AS HE LIFTED IT TO THE MAXIMUM HEIGHT, HE FELT PAIN IN HIS RIGHT SHOULDER. (THE AUGER WEIGHS APPROXIMATELY 80 LBS).

TRUCK DIRVER WAS GETTING INTO TRUCK BED TO DIG OUT CLAY STUCK IN CORNER. AS HE WA GETTING OVER SIDE OF BED HIS HAND SLIPPED AND HE JAMMED HIS FINGER. THE FINGER SWELLED FRIDAY NIGHT AND WAS WORSE SATURDAY MORNING. WENT TO DOCTOR SATURDAY 11/19/88 AND X-RAY FOUND FINGER BROKEN.
EMPLOYEE WAS ASSISTING WELDER DOING WELDING REPAIR TO SCRAPER BOWL. APPARENTLY THE REFLECTION OF WELDER ARC FROM THE SHINY SURFGACE OF THE METAL CAUSED A LIGHT CASE OF WELDER BURN TO THE EYES. EMPLOYEE DID NOT EXPERIENCE PAIN UNTIL THAT NIGHT AFTER WORK.
