Employee was operating an excavator, they felt some pain in left elbow/forearm area but continued working. On Sunday, 5/25/25 EE was at home working on personal vehicle, and felt the same pain, and went to the emergency room to have it check out. Possible left forearm muscle sprain, with doctor advising "avoid activity that causes pain for 7 days".
Employee was in the process of pressure washing windows of the water truck EE was operating, EE started the pressure washer and as EE was backing up while unwinding washer hose/wand, EE attempted to turn around, as EE did, EE tripped over the tongue of the pressure washer trailer, falling to the ground.
Employee was drilling a hole in a 1/2" bolt. As EE was applying pressure to the drill, the drill bit broke, resulting in the pinkie finger of EE's right hand striking the broken drill bit, with a laceration of EE's right pinkie finger.

Employee States on 12/10/24 EE was walking across the parking lot, from the Haul Truck EE had been operating, to assist with repairs on a front-end loader, when EE slipped on ice and fell/twisted Right Knee. Employee Went to ER on 12/19/24, and reported the injury to management on 12/19/24.

Employee was cleaning up around main crushing plant with a skid steer. Went to dump the full skid steer bucket and a 7" rock fell out of the back of the bucket, striking the main frame of skid steer, and then fell into the cab, striking the employee's right knee cap.
Employee strained back while lifting / holding belting on a conveyor, as they were putting new belting on.

Employee stepped up on a 7" platform to check water level in the water tank, completed the check, and as EE stepped back, EE tripped and fell. EE tried to catch self when EE fell, resulting in a left distal fracture to EE's left wrist

Employee was changing v-belt on a conveyor, as EE was attempting to roll new belt onto the pulley, EE's right index finger was pinched between the belt and pulley. Employee decided to have it checked out on 11/2/18 as the fingernail was showing bruising.

A loader operator was feeding a portable screen plant, the loader operator dumped his load into the screen, and started backing up, not realizing another employee in a company pickup had pulled into the area. The loader operator did not see the company truck and backed into it. This resulted in the truck door receiving damage. No Injuries were sustained from this accident.

Employee was performing routine maintenence on crusher, pushing on piece of steel so that it could be bolted into place. Employee felt his back pop and experienced pain. Employee was transported to the emergency room by crusher foreman.
