EMPLOYEE WAS WALKING UNDER #7 CONVEYOR AND WAS STRUCK BY A 4" DIMATER ROCK ON THE HEAD, EMPLOYEE WAS WEARING HARD HAT. THE HARD HAT WA STRUCK CAUSING A LACERATION TO THE RIGHT SIDE ABOVE EAR.

WHEN HE TURNED TO S ET DOWN BAG HE HURT HIS BACK. HE DID NOT TURN HIS WHOLE BODY JUST TURNED HIS UPPER TORSO AND DID NOT MOVE HIS FEET.

EE WAS PUTTING PUMP ON DRILL HAD HAND ON PUUMP. HE WAS TRYING TO PLACE PUMP WHERE IT NEEDED TO GO. HE SLIPPED BACK LOST CONTROL OF PUMP. PUMP CAME DOWN ON TOP OF HIS RIGHT HAND AND INJURED HIS RIGHT RING FINGER.
