EE was climbing down from the operator cab of a loader when he lost his balance and fell to the ground. Was taken to ER on 11/6/11 for x-rays and they were negative. Continued to have pain and a MRI was ordered on 11/15/11. EE was diagnosed on 11/18/11 with a talus fracture.

The hook at the rachet end of the come-a-long came off the beam which allowed the come-a-long to strike employee in the head on the right side of his hard hat. Employee was knocked unconcious. Was taken by air ambulance and then later released.
