EE WAS STANDING BY THE CONTROLS ON THE R-SIDE OF THE MACHINE WHILE THE OPERATOR ON THE LEFT SIDE WAS SETTING THE ATRS. AS THE ATRS WAS PRESSURIZED AGAINST THE TOP. A PIECE OF ROCK 3 1/2 FT. X 5 FT. X 6 IN. FELL FRON THE ATRS BACK TOWARDS THE MACHINE BETWEEN THE R-RIB AND THE BOLTS ON THE R-SIDE STRIKING EMPLOYEE ON THE BACK.

EE HAD JUST PUT THE POWER IN AND TURNED AROUND TO LEAVE WHEN HE TRIPPED ON SOMETHING. HE TRIED TO CATCH HIMSELF WITH HIS RIGHT ARM. WHEN HE DID THIS HE FRACTURED HIS RIGHT ARM.

A FALL OCCURRED IN THE #2 ENTRY OF THE 3 LT. PANEL, THE FALL WAS APPROX. 50 FT LONG BY 19 FT. WIDE BY 7 YO 8 FT THICK THE FALL WAS 1 BK RIGHT OF SPAD 0220 AND 1 BK LEFT OF SPAD 0218. THE EXACT TIME OF THE FALL CAN'T BE DETERMINED DUE TO THEMINE BEING IDLE AT THE TIME OF THE FALL.

A ROOF FALL OCURRED AND MEASURED 100'L X 18'W X 12-15'H. FALL WAS LOCATED IN THE MAINS 1 BK INBYSPAD #138 FROM #2 ENTRY TO #4 ENTRY. FALL WAS HEARD AND LOCATED BY THE 3RD SHIFT FOREMAN WHICH L ET ME DETERMINE THE DATE/TIME. FALL OCCURED ABOVE THE ANCHORAGE POINT.
