EE was ensuring bucket pin did not contain pressure. Employee was using a pry bar to test for movement when the pin moved and the pry bar in a glancing blow struck the employee on the side of the forehead causing a laceration.

Employee was installing v-belts. Employee's right-hand thumb caught between v-belt and pulley and rolled over pulley until free. Employee received five (5) stitches on right-hand thumb and modified work duty.

EE used the fork lift to place a steel wear plate on an excavator bucket to be welded. The plate of steel was placed on the lip of the bucket and needed to be adjusted into place. While pushing into place by hand EE pinched his right thumb causing a laceration requiring medical attention.
Hearing Loss

While hammering a new tooth on a new shank on a 980H a piece of the tooth broke off striking him in the forearm. The metal chip penetrated the skin and lodged about 3 inches up his arm. The employee required 2 stitches and will need to return to the doctor to have the chip removed.

Employee was taking bolts out of front differental of a 988F loader. While pulling down on a breaker bar, the bar broke and hit employee on chin.
Employee failed audiometric testing. Will be retested on 5/4/2009.

Employee was standing on a spring base just peeking over the top of a side plate of a screen looking into the machine. When he started to lower himself, he hyperextended his left shoulder which then dislocated.

HAUL TRUCK DRIVER WAS HAULING WASH SAND TO STOPCKPILE. THERE WAS ADEQUATE BERMING, DRIVER FAILED TO INSPECT FRONT OF STOCKPILE WHERE IT HAD BEEN UNDER MINED. DRIVED BACKED UP TO BERM, THE STOC KPILE GAVE WAY. THE TRUCK WENT HALFWAY DOWN PILE THEN THE TRUCK FLIPPED OVER ONTO ITS ROOF. DRIVER WAS THROWN HALFWAY OUT THE DRIVER DOOR. IT BELIEVE HE WAS NOT WEARING HIS SEATBELT.
