VICTIM WAS OPERATING A 1965 CATEPI LLAR MODEL 46A D-8 BULLDOZER GRATING AN ALLEUVIAL DEPOSIT TO A TWO TO ONE SLOPE HE APPARENTLY LOST CONTROL OF THE MACHINE AND PLUNGED 40 FT TO P IT FLOOR. THE BULLDOZER NOT FITTED WITH ROPS CAME TO REST ON THE CANOPY. SEVERING THE VICTIM. THERE WERE NO WITNESSES TO THE ACCIDENT A TRUCK DRIVER DISCOVERED THE VICTIM NEAR THE SMOLDERING M

Employee was repairing welding gun. The welding gun was in left hand. The left hand was behind screw being removed. Force was applied to screwdriver to remove screw, when the tool slipped from the head of screw. The force being applied caused the tool to puncture left hand.
