EE DID NOT UTILIZE LOTO OR FALL PROTECTION. PER VIDEO & EYEWITNESS, EE WAS WORKING ON ELECTROMAGNET ABOVE CONVEYOR BELT #2. EE STEPPED ON THE BELT FOR THE ELECTROMAGNET THAT TURNS ON EVERY 15 MIN FOR A FEW REVOLUTIONS. THE BELT ENERGIZED AROUND THE TIME EE STEPPED ON IT & THE BELT SWEPT EE OFF THE TOP OF THE ELECTROMAGNET. EE FELL APROX. 20-25 FT STRIKING HEAD & OTHER BODY PARTS.

Removing rust around the plate of the sand screw tub with grinder with 4" wire wheel attachment. Wheel caught the edge and kicked back.

Boro Sand & Stone Corp · Caught in, under or between a moving and a stationary object

Using a 1" airgun to remove a nut from the crushing dies on the crusher. When the nut came off the bolt, the airgun kicked back and caught right hand between it and a structural part of the crusher.

The operator dumped at the bottom of the stock pile all day the previous day.The next day the driver had dumped a load in the middle and admitted to overcoming the berm and the truck went over. The driver exited the truck on his own power.

HE WAS LOADING HIS TRUCK FROM BINS HE WAS STANDING ON A CONCRETE BLOCK WHEN HE STEPPED BACKWARDS HE LOST HIS FOOTING AND FELL AROUND 2 1/2 FT TO GROUND ON HIS BACK.
