WHEN BACKING UP TO BERMED AREA,APPARENTLY OPERATOR MISJUDGED DISTANCE BACKING OVER BERM CAUSING TRUCK TO ROLL AND FLIP,CAUSING OP ERATOR TO BE THROWN FROM CAB. REASON WHY TRUCK AND OPERATOR WENT OVER OR THROUGH BERM IS UNKNOWN.NO ONE OBSERVED TRUCK FROM DUMP AREA.
