HE TOOK A PUNCH AND HAMMER TO HIT CAP AND THE CAP BROKE HAMMER AND HIT AND CUT HIS LEFT THUMB AND LEFT FRONT ON TOP OF HIS HEAD. CUT TO LEFT SIDE OF FOREHEAD AND STITCHES TO THE LEFT THUMB.
WHILE DRIVING A FREIGHTLINER TRUCK HE MADE A TURN TO THE LEFT TO GET ONTO A RAMP AND THE TRUCK LEANED TO THE RIGHT AND UPSET ON RT SIDE. THIS CAUSED HIM TO CRACK A RIB ON HIS LEFT SIDE] OF HIS BACK.
