OPERATOR WAS BACKING INTO DUMP MATERIAL OFF THE CAT 773 TRUCK. WHEN BACKING UP TO BERMED AREA,APPARENTLY OPERATOR MISJUDGED DISTANCE BACKING OVER BERM CAUSING TRUCK TO ROLL AND FLIP,CAUSING OP ERATOR TO BE THROWN FROM CAB. REASON WHY TRUCK AND OPERATOR WENT OVER OR THROUGH BERM IS UNKNOWN.NO ONE OBSERVED TRUCK FROM DUMP AREA.

BACKING UP TO RIB SANDROCK THAT WAS SHOT, JARRING AND RIPPING FOR APPROX 6 HRS. SUDDENLY HE FELT A SHARP PAIN IN LOWER BACK, SEVERE ENOUGH TO CAUSE HIM TO STOP FOR A FEW MINUTES. PAIN CONTINUE D THROUGHOUT THE SHIFT.

REMOVING DRIVE SHAFT FROM FRONT REAR END ON MACK TRI-AXLE. WHILE DOING THIS, HE WAS USING A BAR TO PRY SHAFT AND I SLIPPED AND HIT HIS HEAD. HE TOOK A PUNCH AND HAMMER TO HIT CAP AND THE CAP BROKE HAMMER AND HIT AND CUT HIS LEFT THUMB AND LEFT FRONT ON TOP OF HIS HEAD. CUT TO LEFT SIDE OF FOREHEAD AND STITCHES TO THE LEFT THUMB.
