Mechanic was replacing a 6? X 5? spool of pipe on the final concentrate pump discharge. Employee had to use come along to line pipe up. After job completion and while removing come along chain from around pipe and as hook came over pipe the hook fell to lower pipe, striking employee right thumb. X-ray revealed fracture.

While walking down the stairway a piece of out of service pipe fell and laid across handrail. Crane was called to pick up pipe as it was suspended above another working area. While picking up pipe, employees, thinking the pipe was wedged, let down load. While shaking load employee's left index finger got pinched between pipe and screen rail.

Employees were attempting to open a 6" gate valve. Pipe wrench was applied to handle of valve to increase leverage; employee grabbed valve wheel as other employee pulled on pipe wrench. Little finger on right hand was caught in pinch point.

Mechanic was assigned the task of welding a cable retention device on a water jack pontoon. The mechanic was positioned on top of the pontoon for part of this task. The pontoon blew open the top seam on the north side, knocking the mechanic up in the air and backward appproximately 15 feet southward onto the concrete slab.

EE was climbing down into the hole of the revolving frame without the use of a ladder to weld a seam. When he got to the bottom, his (L) knee gave out on him. Reportable as of 8/25/05. EE did not seek medical treatment until 8/25/05 (1st visit to doctor): severe knee strain requiring the use of an immobilizer, crutches and sedentary work only.
