VICTIM WAS RIDING CAGE FROM BOTTOM LOADING LEVEL TO SURFACE WHEN FOR SOME UNKNOWN REASON HE STUCK HIS HEAD OUT OF THE CAGE AND HIS HEAD WAS CAUGHT BETWEEN THE HOIST CONVEYANCE AND THE SHAFT TU MBER
