THE VICTIM WAS 8TRUCK BY A PIECE OF LOOSE COAL RIB ON THE LONGWALL SECTION NEXT TO THE STAGE LOADER, CAUSING HIM TO STRIKE HISHEAD AGAINST A METAL FLANGE ON THE BARREL OF THE SPEED REDUCER-CAU SING A SEVERE H9AD INJURY.
