THE VICTIM WAS IN THE PROCESS OF DRILLING A ROOF BOLT HOLE WHEN HIS HEAD WAS CAUGHT BETWEEN THE STATIONARY CANOPY AND THE MOVING DRILL HEAD BOOM OF THE BOTLING MACHINE. THE VICTIM SUSTAINED FA TAL MULTIPLE-TRAUMA HEAD INJURIES.
