VICTIM WALKED INTO THE #3 ENTRY LEFT CROSSCUT TO TALK TO ROOF BOLTER OPERATORS. AFTER A BRIEF CONVERSATION WITH ONE OF THE OPERATORS, HE WALKED INTO THE #3 HEADING. A ROOF FALL OCCURRED WHIC H EXTENDED PAST THE PERMANENT SUPPORT FATALLY INJURING THE VICTIM.
