EMPLOYEE FAILED TO FOLLOW ESTABLISHED PROCEDURE OF DUMPING AT BOTTOM OF UNDERCUT PILES. EMPLOYEE BACKED TRUCK ONTO AN UNDERCUT PILE RESULTING IN TRUCK ROLLING OVER FATALLY INJURING DRIVER. EMP LOYEE REPORTED TO WORK AT 8:40 A.M. AFTER WORKING A 4:00 P.M. TO 11:30 SHIFT AT HIS OTHER PLACE OF EMPLOYMENT.
