TRANSFORMER LOCATED OUTSIDE NEWLY CONSTRUCT ED SURFACE SHOP FOR UNDERGROUND ON 2330 BENCH OFPIT. CONTRACTOR'S EMP FOUND VICTIM LAYING ON GROUND NEXT TO TRANSFORMER. DOORS OF TRANSFORMER COMPARTMENT OPEN, EXPOSING 4160 & 480 VOLT TERMIN
