TWO EMPLOYEES WERE CHANGING OUT A HOIST CYLINDER ON A CLARK FRONTEND LOADER WHEN HE CRAWLED BETWEEN THE BOOM ARMS OF THE LOADER (WITHOUT SUPPORTING THE BOOM) AND REMOVED THE PLUG FROM THE HYD. LINE ALLOWING THE BUCKET TO FALL PINNING HIM BETWEEN THE BUCKET AND THE SHOP FLOOR, RESULTING IN DEATH.
