Struck by falling object

The employee was loading steel from a steel storage rack onto a truck-mounted overhead rack. A 23' by 1 1/2"" angle iron fell from the truck-mounted rack striking him on head and neck. He was taken to William W. Backus Hospital for treatment.

While removing a bolt used to attach the cutting edge to a loader bucket, employee thought bolt was dropping out of hole. He put his hand under to catch it, and the spinning bolt cut his fingers.

EMPLOYEES WERE IN THE PROCESS OF DISMANTLING CONVEYER. THE EMPLOYEE WAS IN THE PROCESS OF CUTTING A PIECE OF ANGLE IRON WHEN THE CONVEYER DROPPED ABOUT 6-12 INCHES STRIKING THE EMPLOYEE IN THE LEG ABOVE THE KNEE RESULTING IN A LACERATIONNEEDING MULTIPLE STITCHES.

EE WAS WORKING WITH 2 COWORKERS TO TAKE APART A LARGE FEED HOPPER. WHILE THE INJURED EE WAS WAITING FOR ADDITIONAL TOOLS, TO ARRIVE TO COMPLETE THE PROJECT HE BECAME IMPATIENT AND BEGAN TO COM PLETE THE WORK HIMSELF. CUTTTING THE BOLTS THAT HELD THE HOPPER IN PLACE, HE CUT THE FINAL BOLT ASSUMING THE HOPPER WOULD FALL STRAIGHT DOWN. INSTEAD, IT SHIFTED AND LANDED ON HIS FOOT.

WHILE RECEIVING ASSISTANCE FROM 2 CO-WORKERS TO PLACE A 20' SECTION OF CATWALK. THE CATWALK MOVED AND PINCHED EMPLOYEE'S HAND BETWEEN THE CATWALK AND A CEMENT BLOCK REQUIRING STITCHES.
