According to hearing tests performed it has been determined that employee has sustained a hearing loss threshold shift in left ear.

According to hearing tests performed, it has been determined that employee has sustained a hearing loss threshold shift in both ears.

Mechanic was replacing a 6? X 5? spool of pipe on the final concentrate pump discharge. Employee had to use come along to line pipe up. After job completion and while removing come along chain from around pipe and as hook came over pipe the hook fell to lower pipe, striking employee right thumb. X-ray revealed fracture.

While walking down the stairway a piece of out of service pipe fell and laid across handrail. Crane was called to pick up pipe as it was suspended above another working area. While picking up pipe, employees, thinking the pipe was wedged, let down load. While shaking load employee's left index finger got pinched between pipe and screen rail.

Employee was stepping off last step of float plant onto concrete floor when foot slipped causing employee to fall, resulting in injury to left wrist.

Employees were attempting to open a 6"" gate valve. Pipe wrench was applied to handle of valve to increase leverage; employee grabbed valve wheel as other employee pulled on pipe wrench. Little finger on right hand was caught in pinch point.

Mechanic was assigned the task of welding a cable retention device on a water jack pontoon. The mechanic was positioned on top of the pontoon for part of this task. The pontoon blew open the top seam on the north side, knocking the mechanic up in the air and backward appproximately 15 feet southward onto the concrete slab.

Employee was operating payloader. He parked the loader to get off. While coming down the ladder the employee missed the last step falling to the ground and injuring his right wrist.

Employee put on a pair of used leather palm work gloves. When he removed the gloves he complained of his hand itching and then hives appeared on his wrists.

EE WAS SHOVELING AROUND HOSE FLANGES WHEN HE BEGAN TO FEEL OVERHEATED.

EE was climbing down into the hole of the revolving frame without the use of a ladder to weld a seam. When he got to the bottom, his (L) knee gave out on him. Reportable as of 8/25/05. EE did not seek medical treatment until 8/25/05 (1st visit to doctor): severe knee strain requiring the use of an immobilizer, crutches and sedentary work only.

EMPLOYEE STATES THAT HE BENT OVER TO PICK UP TOOL AND FELT PAIN IN BACK.

EE WAS USING WRENCH TO LOOSEN BOLT AND SMASHED HIS FINGER BETWEEN THE WRENCH AND PIPE.

Employee states he was wiping face with rag and apparently a piece of dirt or sand scratched his eye. He was given Rx eye drops.

Employee tested positive for STS Hearing Loss.

EMPLOYEE STATES HE WAS WORKING ON BEARING ASSEMBLY WHEN WORKER BEHIND HIM TURNED AROUND AND OTHER WORKER'S TORCH BURNED INJURED EMPLOYEE ON BACK OF HEAD & EAR.

EE STATES HE WAS REMOVING BOLTS WITH AN IMPACT WRENCH AND SLIPPED, CAUSING HIM TO PINCH THUMB BETWEEN WRENCH AND PIPE.

EE WAS STEPPING BACKWARDS AND TRIPPED OVER A MOTOR STARTER CAN CAUSING HIM TO FALL ONTO ANOTHER MOTOR STARTER CAN.

EE WAS USING A 5' PRYBAR TO REMOVE CROSS TIE SPIKE FROM RAILROAD TRACK, AS HE PUSH DOWNWARD, THE PRYBAR SLIPPED AND HE PINCHED FINGER BETWEEN BAR AND GROUND. HE WAS WEARING GLOVES. EE DID NOT FOLLOW INSTRUCTIONS TO KEEP FINGERS FROM UNDERSIDE OF BAR, TOOL NOT DEFECTIVE AND WEATHER WAS GOOD.

EE ALLEGES THAT WHILE WALKING ALONGSIDE PIPE HE SLIPPED & TWISTED ANKLE. (HAS BEEN UNDER INVESTIGATION.)

EE DROVE 50' OFF BOARDERED HAUL RD, HIT A WASHOUT APPROX. 2' DEEP 3' WIDE, SAID SEATBELT HURT ADBOMEN.

EE WAS USING RATCHET TO UNBOLT PIPELINE. AFTER HE FINISHED JOB HE WAS WALKING AWAY. HE STATED HE FELT SHARP PAIN IN BACK WHILE WALKING. INCIDENT HAS BEEN UNDER INVESTIGATION. RECEIVED PRESCRIP TION MEDICINE.

EMPLOYEE STATES HE WAS USING TORCH WHEN HOT SLAG BURNED THROUGH TOP OF BOOT INTO FOOT. INITIALLYDIAGNOSED AS 1ST DEGREE BURN. BURN LATER WORSENED AND GOT INFECTED RESULTING IN MAN GETTING PRESCRIPTION MEDICINE. INFORMATION JUST RECENTLYTOLD TO SAFETY DEPT.

EMPLOYEE STATES HE WAS WORKING ON PACKING ON PUMP. LINE HAD BEEN PLUGGED CAUSING WATER TO RE-CIRCULATE INSIDE PUMP AND GET HOT. WHEN HE OPENED PACKING, HOT WATER GOT ON FACE AND NECK, CAUSING 1ST DEG. BURN, SMALL SPOT ON NECK BLISTERED RESULTING IN 2ND DEG. BURN.

EE STATES THAT WHILE PULLING CABLE HE FELT PAIN IN ARM AND SHOULDER. ON MAY 9 HE WAS GIVEN PRESCRIPTION MEDICINE AND PALCED ON RESTRICTED DUTY./

EE STATES HE WAS LOOSENING BOLT & WRENCH SLIPPED CAUSING HIM TO CUT HIS HAND ON PIECE OF STAIR METAL OVERHEAD.

EMPLOYEE WAS USING A PRYBAR WHICH SLIPPED AND HEFELL BACKWARDS FRACTURING HIS RIGHT ANKLE.

EMPLOYEE STATES THAT HE WAS USING PRY BAR AND PINCHED FINGER BETWEEN BAR AND STEEL PLATE. ON 7/12/00 HE WAS GIVEN SPLINT ON FINGER. (REPORTED 7/7/00)

EE STATES HE WAS USING A PRYBAR AND FELT A PAIN IN HIS SHOULDER. GIVEN PRESCRIPTION AND RESTRICTED DUTY.
